Issaquah School District 411 FACILITIES SERVICES The District supplies this Performance Improvement Checklist when an employee’s performance is determined to be unsatisfactory for any given section of their annual Evaluation. Every effort will be made to provide training and support necessary for the successful completion of this Performance Improvement Checklist. Improvement must occur immediately and must be maintained. The process of performance improvement provides that if management determines at any point during or after this time period that your overall performance is not significantly improving, additional performance deficiencies develop, or performance regresses, disciplinary action will be taken, up to and including termination. A decrease in performance after successfully completing the Performance Improvement Checklist may result in being dismissed without the issuance of another warning or improvement plan. The Weekly Work Log will be reviewed by employee and supervisor weekly to identify areas where problems are occurring and to devise alternative plans when necessary. The checklist will be reviewed by employee and supervisor weekly to identify areas where problems are occurring and to devise alternative plans when necessary. 3. _____ HUMAN RELATIONS Goals and Timetables: Employee will participate in individualized training provided by Personnel Services to be scheduled for no later than ____________, 2013. In addition, the employee will have access to $300 for off site training pre-approved by the direct supervisor to be used no later than ________, 2013. Action Plan: Employee will verbally share with supervisor weekly examples of an attitude of cooperation, willingness, flexibility, and other positive reactions. Employee will not have substantiated reports of violation of ISD Civility Policy and or Harassment Policy. 4. _____ PROBLEM SOLVING AND INITIATIVE Goals and Timetables: Employee will participate in individualized training from their supervisor, focusing on hypothetical workplace scenarios and appropriate responses. This is meant to be interactive, and not assessment. Action Plan: Employee will verbally share with supervisor weekly examples of being able to make necessary changes in the way a task is done and being able to cope with and respond to unexpected events. When priority assignments overlap, you will bring work with due dates appropriately identified to your supervisor. Your supervisor will consult with the individuals generating the work and arrange to amend the priority of the work to be performed.
