Safe Graduation 2015
Dear Parents/Guardians,
The E.O. Smith Safe Graduation Committee is finalizing plans for this year's event on
June 17th following graduation. Safe Grad 2015 is a late-night drug and alcohol free
chaperoned evening which provides our graduating seniors with a memorable celebration. For
everyone's safety, we invite and encourage all seniors to attend the celebration.
Safe Grad 2015 will be held at the Mansfield Community Center in Mansfield on June
17th from 10:30-4:00 am. Students need to arrive at the community center for registration
between 10:30 pm and 11:30 pm.
There can be no admittance after 11:30 pm. Planned activities include a hypnotist,
DJ, airbrush tattoo artistry, photo booth, Texas Hold'em, laser tag, ping pong, pool and
swimming. A late night buffet and early morning continental breakfast will be catered by local
restaurants. Each senior who attends and remains for the entire evening is eligible for a
chance to win a wide variety of prizes. The raffle is the last event of the evening. Students
must be present to win a raffle prize.
As you can imagine an event of this magnitude depends on parental support. Please
take the time to read and sign the enclosed contract. To ensure the safety of all students,
absolutely no exceptions to these rules will be allowed. Tickets for the event are $40.00 if
fee and contract are received by June 1st. Any contract received after June 1st will be
$45.00. It is our goal to have all seniors attend. If the cost is a hardship for you, please
contact the guidance office at E.O. Smith.
To ensure the safety of the graduates, we encourage the students to attend the entire
event. If for any reason your child requests to leave early, we will call you at that time to notify
you of their departure. Because students will be tired at the end of the event, we recommend
that you make appropriate transportation plans for the ride home. No written notes for early
release will be accepted. Please discuss this with your child.

Student Participation Contract
I will be attending the E.O. Smith Safe Grad celebration at the Mansfield Community Center
on June 17th from 10:30 pm until 4:00 am. My signature below indicates that I agree to abide
by the following conditions, without exception.








I will not drink alcohol or use any other illegal substances while attending E.O. Smith Safe
Grad 2015
I understand that there will be no smoking permitted inside or outside of the community center.
I understand that food and drink will be provided and that except for conditions of food allergies,
absolutely no food will be allowed to be brought into the community center.
I understand that there will be no early departures from the building. Parents will be notified if a
student requests to leave for any reason, medical or otherwise.
I will be present to register from 10:30 pm to 11:30 pm and will be in the building by 11:30pm. I
understand that if I have registered and do not arrive for Safe Grad, my parents will be called to
verify my whereabouts.
I understand that I must be present at the celebration for its entirety to be eligible to win a
prize.
In case of emergency, my parent/guardian gives his/her permission for me to receive immediate
medical attention at the nearest hospital. My parent/guardian will be notified as soon as
possible.
I agree to have my personal effects (all bags and backpacks) searched upon admittance to the
event.

Project Graduation Facility Use Waiver
WAIVER OF PARTICIPANT/FACILITY USER BY PARENT OR SELF: I hereby agree to release, discharge and hold
harmless The Town of Mansfield, its directors, officers, employees, agents, contractors and/or volunteers from any and all
liability or damage that may occur during my participation or the participation of my minor child in activities at or use of the
Mansfield Community Center facility. I understand that participation in any recreational or sport activity involves risk, and I
grant permission to the Town of Mansfield to utilize any medical emergency services it deems necessary to treat any injuries
that I, or my child, may incur. I further understand that the Town of Mansfield does not provide insurance for recreational
program participants and facility users. Photo Release: I understand that for promotional purposes, the Town of Mansfield
videotapes and /or takes photographs of participants and facility users. I hereby release and permit the Town of Mansfield to
utilize for said promotional purposes any videotapes and /or photographs of my child or me engaged in facility activities and
/or general facility use.
