AGREEMENT TO PROVIDE RESIDENTIAL HOUSING
ASSISTANCE FOR VICTIMS OF HURRICANE
I understand that the City of Olsonville (the "City") is entering into a residential lease
agreement on my behalf as an "Evacuee" or
"Eligible
Shelter Victim" under any sponsorship or program sponsored by FEMA. The City has
agreed to provide the following assistance in obtaining the residential lease:

The City will enter into a lease for the residential premises through __________ , 20

The City will pay the Security Deposit, if applicable, for the residential
premise;

The City will pay the electrical, gas, and water bills incurred during the
term of the residential lease;

The City may rent or buy furniture for your use during the term of the lease
for the residential premise; and

In addition, the lease will terminate in the event:

You abandon or vacate the leased residential premise;
You leave Olsonville, Texas;
You no longer retain registration or the status of "Evacuee" or
"Eligible Shelter victim" under any sponsorship or program
sponsored by FEMA; or
You notify authorized representatives of the City or FEMA that you
no longer wish to participate or remain registered in any
sponsorship or program sponsored by FEMA.
