Training of improvement facilitators to support the goal of Medical Home is being offered and supported by Toward Optimized Practice. The delivery of primary care through a Medical Home model will see patients “attached” to a care provider/team and their care managed through optimized utilization of EMR to ensure care delivery is timely and appropriate, that care is delivered by the pertinent team member and that care coordination within and outside of the team is timely and appropriate. C2 It is expected that these improvements will lead to the encouragement of healthy lifestyles and habits, enhanced chronic disease management and preventive health screening, leading ultimately to early detection of significant health issues. Position summary: The Improvement Facilitator will report to name position within employing organization and will be supported in the work by a TOP Clinical Process Advisor, and other team members through TOP. The Improvement Facilitator plays a key role in implementing the initiative in practices that have expressed interest in participation. This will include, but is not limited to, assisting practices with understanding their current access for care, their processes for patient attachment, their model of care delivery, the use of EMR to support the maintenance of the patient panel and to guide clinical care improvement initiatives based on accurate EMR data. The facilitator will assist with the identification of improvement opportunities; developing customized work processes for improved patient care using principles of advanced access, panel management; mentoring and coaching clinic staff to implement new/redesigned processes; supporting efforts to maximize the use of the EMR/or other documentation system in identifying patients who require care intervention, care coordination or preventive health screening (with support from TOP); collecting standard reports on progress and spreading the initiative to other practices within the name employer. This individual may be required to engage & develop relationships with primary health care teams and family physicians to outline the initiative advantages and processes. 2 QUALIFICATIONS: Education: Undergraduate degree or a diploma in a related field of study, preferably a health discipline. Experience: Experience, knowledge or additional training in change management, improvement methods and QI Principles and their practical application as related to primary care is needed (training in these areas will be provided by TOP). Previous experience as a coach/mentor and/or facilitator and experience working with family physician, primary care teams and/or other primary care practices is preferred. A combination of education, experience and expertise may be considered. Skills: Develop rapport with health providers. Communicate effectively and constructively, orally and in writing. Proficiency in using Microsoft Office applications. Understanding of primary health care delivery organizations (including family physician practices) in Alberta. Demonstrated ability to manage change. Be able to work independently and as part of a team. Work in a professional and confidential manner, displaying a high level of integrity and respect for others. Assist teams to achieve goals through facilitation, problem solving, decision making and conflict management skills. o Support ongoing team meetings, reinforcing initiative purpose, methodologies and concepts. o Assist teams in identifying roles of each that will promote efficiency and maximize care delivery to patients reached through the initiative. o Facilitate strategies that will enable teams to gather data and share outcome and process measures. o Promote knowledge transfer related to tests of change. o Assist teams in identifying and overcoming obstacles during the preparation phase, implementation, and sustainability phases of the initiative. o Provide group facilitation to achieve shared objectives. o Implement plans relating to the initiative including understanding the current clinic practices, optimizing the use of EMRs with the practice, expanding the scope of the team to meet initiative goals, setting up processes to report as required, supported by the TOP Clinical Process Advisor and other TOP team members o Support clinic staff as the initiative is tailored to the specific processes of each practice. Submit standardized reports on progress and outcomes as required by the initiative.
