Applications submitted for consideration of membership in the NJHS with any missing, incomplete or
inaccurate information will be considered incomplete and will be returned to the candidate as soon as
possible. Applicants may resubmit their paperwork for consideration as long as the submission is
within the timeline established by the adviser. No deadlines for submission will be extended. Please fill
out the form as neatly as possible. Proofread and make sure everything is filled out when you turn it in.
I have read and to the best of my knowledge have completed the attached information packet. I
understand that retuning this information packet puts my name in consideration for Membership but
does not guarantee an invitation to become a member.
I do hereby give my consent that any and all written records created, used or maintained by the School
in connection with the process of selection of candidates to become a member of the Noblesville West
Middle School National Junior Honor Society shall remain confidential from my child and myself, and I
hereby waive and give up any and all rights that I have under the Indiana Public Records Act, or any
other statue, ordinance or law to gain access to copies of such records or materials.

Leadership Recommendation for NJHS
Leadership can manifest itself in different ways. We are specifically looking for situations where a
student possesses a positive attitude and where an adult observed that student and can attest to the
student’s abilities as a self-starter, group leader, problem solver, effective communicator, etc. While a
student may not possess all the above qualities, the recommendation should be able to demonstrate
evidence of these qualities in the student. This form can be filled out by an adult working in or outside of
the school, but not by a member of the student’s family. The NJHS council will review all applications and
letters and use their discretion as to whether or not leadership has been demonstrated.

“Give examples of situations in which you have experienced this student exhibiting positive, hard
working behavior. Whether it is in a class, on a team or in an organization. Please include if the student
has any problem solving skills, communication skills, keeps a positive attitude and delegates
