Section 73.3615(d)(3) of the FCC’s rules require
that an ownership report on FCC Form 323-E be
submitted to the FCC biennially commencing on
the station’s renewal application filing date
Sections 73.1020 and 73.3539 of the FCC’s rules
requires that a renewal application on FCC Form
303 be submitted to the FCC every eight (8)
years. For Ohio radio stations, the next renewal
application will be due on June 1, 2012.
Section 73.2080(f)(1) of the FCC’s rules requires
that an EEO Program Report on FCC Form 396
be submitted to the FCC (including the last two
years of Annual EEO Public File Reports [see
below]) along with the filing of the renewal
application for the station. Every licensee must
submit this report to the FCC regardless of
whether they employee less than five (5) full-time
employees.
Section 73.3612 of the FCC’s rules requires that
an Annual Employment Report on FCC Form
395-B be submitted to the FCC every year on
September 30th reporting data from one payroll
period in July, August, or September. The same
payroll period should be used each year.
Please note that as of December 20, 2004, the
filing of the Annual Employment Report on FCC
Form 395-B is suspended until the FCC releases

Employers are not required to keep OSHA injury
and illness records for any establishment
classified in the following Standard Industrial
Classification (SIC) codes (including educational
institutions), unless they are asked in writing to do
so by OSHA, the Bureau of Labor Statistics
(BLS), or a state agency operating under the
authority of OSHA or the BLS. All employers,
including those partially exempted by reason of
company size or industry classification, must
report to OSHA any workplace incident that
results in a fatality or the hospitalization of three
or more employees (see § 1904.39).
