Please help us:

 Treat staff with courtesy and respect. Reception staff may have to ask some personal questions to assist in providing you with the best service

 Attending your appointment on time, letting us know if you intend to cancel an appointment or are running late. Cancelling early enables us to use the time for another patient

 Inform the practice staff of any alterations in your circumstances, such as change of surname, address or telephone number

 Making a separate appointment for each member of the family

 Being prepared to wait if you request an appointment for an urgent problem which requires treatment on the same day

 As patients, you are responsible for your own health and that of any dependents.

It is important that you adhere to information and advice given to you by health professionals, and co-operate with the practice in endeavoring to keep you healthy.

There are a number of appointments available for each GP Monday to Friday that can be pre-booked up to 4 -6 weeks in advance; the remaining can be booked from 8am the same day. Appointments can also be booked with the Doctors or Nurse Practitioner online. In addition, patients are able to cancel an appointment online via the practice website.

Responsible for management of staff, administration of practice finance, organisation, health and safety, management of premises, non-clinical patient welfare, complaints liaison manager and computer manager.

Transcribing all referral letters, forwarding them via Choose & Book, patient/hospital queries, hospital DNA letters and insurance reports. Leanne is minute clerk for in-house meetings, also responsible for immunisation recalls and targets, minor surgery and cryotherapy clinics.

The Health Care Assistants role includes phlebotomy, blood pressure, ECG taking, spirometry, dressings, weight management, smoking cessation and Healthy Heart Checks.

Phlebotomist: Leanne Kirwin: Qualified to administer the taking of bloods, BP readings and ECG’s.

Promoting Practice Effectiveness - Nurse Practitioner Pathway

Can treat a range of conditions and can prescribe many medications. Also provide telephone advices.

Please be aware that Nurse Practitioners are unable to see children under 12 months and pregnant ladies.

All Registered Nurses (RGN qualified) offer advice on minor problems and who run many of the clinics mentioned in this booklet.

District Nurses:

There is a team of nurses working in the community to care for the medical needs of housebound patients

Midwife:

Attends to the needs of expectant and newly delivered mothers and their babies

Physiotherapist:

Patients can be referred for physiotherapy through their GP.

Accident & Emergency

Your local Accident & Emergency Department is at The James Cook University Hospital, Marton Road,

Middlesbrough. This deals with injuries and life threatening emergencies.

Ambulance Bookings

Ambulance transport to Hospital Out Patients is intended for people with disabilities or housebound patients who cannot get to hospital by any other means. If you need to an ambulance please ring 0345 045 0160.

Antenatal Clinic elderly book

Maternity services are available from doctors and midwifes. You will be asked to come to surgery for an 8 week postnatal check.

Chronic Disease Management

Our nurses offer check-ups of chronic disease including asthma, diabetes and Coronary Heart Disease. These are intended to keep our patients at their peak fitness. Our specialist nurses teach you about your condition and check that you are getting the best from your prescribed medication.

Complaints

We make every effort to give the best service possible to everyone who attends our practice. However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible. Please contact the Practice

Manager and she will set all the necessary wheels in motion. Further written information is available on the complaints procedure from Reception. We are continually striving to improve our service, any helpful suggestions would be much appreciated and a suggestion box is located in the waiting area.

Family Planning - Family planning services and advice can be given by doctors, nurses and nurse practitioners.

Cervical smear tests are taken by the nurse by appointment only.

Health and Safety - The practice aims to provide safe and healthy premises for staff and patients. Please behave in a responsible and reasonable manner while you are on the premises. Equipment must not be touched or used without instruction or supervision by staff.

Interpreters - If you are a patient with difficulties in speaking English, the surgery staff are able to arrange for an interpreter to attend your appointment with you to interpret for you.

Minor Surgery/Cryotherapy and Joint Injection Clinics - Minor surgery and Cryotherapy is offered to patients with warts, moles, cysts or skin tags. Joint injections are also available. Please ask your GP or Nurse Practitioner to refer you if appropriate to our in-house clinic for Dr Hameed or Dr Sree

Physiotherapy - Clinics are held for back care advice, joint mobilisation, acupuncture and general advice and exercise. Your doctor can refer you if appropriate.

Podiatry - This service is provided at the surgery by One Life. The Podiatrist is responsible for taking care of patient’s feet

Sexually Transmitted Diseases - If you prefer, these can be treated in James Cook University Hospital by a trained specialist, who will also give advice. Please telephone the Centre for Clinical Infection on 01642 835677 for an appointment. Treatment and advice is given in strict confidence.

Smoking Cessation Clinics - Please arrange an appointment with one of our Health Care Assistants for advice on giving up smoking.

Travel Information - If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. Anti-malarial are not available on the

NHS. You can buy directly from the pharmacy. Remember to continue anti-malarial for up to 4 weeks after your return to the UK.

Treatment Room - Our practice nurses undertake dressings, remove stitches, syringe ears, take blood samples, carry out ECG’s and cervical smears, give childhood and travel vaccinations and run various health clinics.

Well Person Check - Simple checks and tests can often identify preventable health problems. This clinic will check your general fitness and give advice on staying healthy. You will also be given advice on preventing heart disease.

APPOINTMENTS:

The practice operates an advanced access system to assist patients in gaining timely appointments with a healthcare professional. Should you like an early morning appointment or a late appointment, these appointments can be pre-booked up to 4 weeks in advance. For all other appointments please ring on the day you wish to attend from 8.00am.

How to register as a new patient:

To register at the practice please ask at reception. The receptionist will provide you with a registration form and will invite you for a health check with a member of the nursing team.

Seeing the Doctor:

Consultations are by appointment only and can be made through our advanced access appointment system or on the practice website. We would encourage you to see the same Doctor as far as possible for continuity of medical care. All patients have the right to express which practitioner they would prefer to see. If you require urgent medical attention, we will always see you on the same day, although you may sometimes have to wait to see another Doctor.

Nurse Practitioner:

She is available to treat a range of conditions. For example; asthma, skin conditions, women’s health and contraception, coughs and colds, hay fever, earache, sore throat and HRT. Please be aware that Nurse Practitioner is unable to see children under 12 months and pregnant ladies.

Appointments can also be made to see our Practice Nurses and Health Care Assistants as needed in advance.

Home Visits

If possible please try to telephone before 10am if you require a home visit. A doctor may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance. House visits are only available for patients who are housebound because of illness or disability. Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.

CONFIDENTIALITY

The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others:

- To provide further medical treatment for e.g. from district nurses/hospital services.

- To help you get other services e.g. social work department (requires your consent).

- When we have a duty to others e.g. in child protection cases, anonymous patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know. Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

FREEDOM OF INFORMATION ACT

The ICO has published a new Model Publication Scheme that all public authorities are required to adopt.

CCTV

The practice operates CCTV both in the practice and the car park to ensure security and safety of our patients and staff.

DISABLED FACILITIES

The surgery has suitable access for disabled patients. All the patient areas including the waiting room, the consulting rooms and the toilets, have wheelchair access. A hearing loop is in place at the front reception desk. If you require one, there is a wheelchair available at reception. There are designated disabled parking spaces in the car park.

Out of Hours

If you need urgent medical attention when the surgery is closed, please telephone NHS 111. The out of hours service is not provided by the surgery, but is commissioned NHS England. You may be asked to attend a Primary

Care Centre and, as in the day time, will only be visited if the doctor considers it medically necessary.

Telephone Advice Line

If you have a medical query which you feel may not require a doctor’s appointment, our advice line is available.

Please phone the surgery before 11.00am to request telephone advice and leave a contact number. A Doctor or

Nurse Practitioner will phone you back.

Repeat Prescriptions

To request a repeat prescription, patients must give a minimum of 48 hours notice. Please be aware that any requests submitted out of hours (between 6pm - 8am, weekends or bank holidays), will not be processed until the following working day.

VIOLENCE STATEMENT

The practice considers aggressive behaviour to be any personal, abusive and/or aggressive comments, cursing and/or swearing, physical contact and/or aggressive gestures. The practice will request the removal of any patient from the practice list who is aggressive or abusive towards a doctor, member of staff, other patient, or who damages property.

All instances of actual physical abuse on any doctor or member of staff, by a patient or their relatives will be reported to the police as an assault.

For those who have had their normal childhood immunisations, which include tetanus, a tetanus booster may be recommended if patients incur a wound.

COMMON ILLNESSES-SELF HELP

Cold & Flu - Most last for only a few days and get better without treatment. They often start with a runny nose, cough, temperature and aches. They are caused by viruses. If the symptoms are distressing or uncomfortable, the treatment consists of taking the recommended dose of Paracetamol and drinking plenty of fluids.

Diarrhoea & Vomiting - This is normally caused by an infection in the stomach or bowel. Although the diarrhoea and vomiting may be unpleasant and uncomfortable, they are nature’s way of flushing the infection from the system. It is better not to take any medicine or tablets but to drink plenty of fluids. Diarrhoea and vomiting in very young children should be discussed with your Doctor in person or via the telephone advice line which is available at the surgery.

Burns & Scalds - Taking the heat away from the skin as soon as possible is the most important factor. Apply large quantities of cold water immediately, for up to 15 minutes if necessary. Do this before any other measure such as removing clothes or sending for an ambulance or Doctor. If the skin is broken but blistered, apply a large dressing after cooling. If the skin is broken, or larger than 4 inches in diameter, contact the local A&E department.

Head Lice - These are not a sign of poor hygiene. Shampoos and lotions which can relieve the problems are available from the pharmacist without prescription.

Backache & Strains - Many acute strains and backache will respond to a few days rest, with

Paracetamol taken for pain.

Sprains - First apply a cold compress containing ice for 15-30 minutes to reduce the swelling. Apply a firm crepe bandage and give the sprain plenty of rest until all the discomfort has subsided.

Insect Bites & Stings - Most of these need no treatment. Antihistamine tablets can be obtained from the pharmacist without prescription and will relieve most symptoms.

Threadworm - These usually manifest as itching at the bottom and treatment can be obtained from the pharmacist without prescription. All members of the household (more than 3 months of age) should be treated. Treatment may be ineffective unless strict attention is pain to personal hygiene, in particular hands should be washed and nails scrubbed before meals.

Nose Bleeds - Sit in a chair leaning forward with your mouth open. Pinch your nose just below the bone continuously for at least 10 minutes, by which time the bleeding will probably have stopped. If the bleeding continues consult your doctor or, out of hours, go to the nearest A&E department

Sunburn - Treat as for other burns with cold water to remove the heat. Lotion with relieve the irritation and

Paracetamol will help. If you are badly burn consult your doctor. To help prevent sunburn, make sure that you apply a good quality sunscreen to all exposed parts of your body. Re-apply the sunscreen every two hours when outdoors, especially if you are swimming or sweating.
