PRACTICE OPENING TIMES
Monday - Friday
8.00am – 6.00pm Closed 1 – 2pm Thursdays for staff meeting.
Saturday
8.00am – 11.00am (subject to change)
A receptionist is available between these times to take your calls.
For any routine queries (for example to check if any letters/results have been received),
we ask that you call after 2.00pm.
Patient Rights
You will be treated with respect and as a partner in your care. Being a partner means
you have responsibilities too.
We will:
 Ensure the surgery is clean, comfortable and easy to access
 Respect our patient’s confidentiality, and whenever necessary, provide a private
place to speak to staff
 Be helpful and polite
 Treat you as an individual regardless of your ethnic origin, religion, sexual
orientation or illness
 Ensure our patients have 24-hour access to medical advice
 Work in partnership with you to achieve the best medical care possible. The
prevention of disease, illness and injury is a primary concern
Please help us:
 Treat staff with courtesy and respect. Reception staff may have to ask some
personal questions to assist in providing you with the best service
 Attending your appointment on time, letting us know if you intend to cancel an
appointment or are running late. Cancelling early enables us to use the time for
another patient
 Inform the practice staff of any alterations in your circumstances, such as change
of surname, address or telephone number
 Making a separate appointment for each member of the family
 Being prepared to wait if you request an appointment for an urgent problem
which requires treatment on the same day
 As patients, you are responsible for your own health and that of any dependents.
It is important that you adhere to information and advice given to you by health
professionals, and co-operate with the practice in endeavoring to keep you
healthy.

There are a number of appointments available for each GP Monday to Friday that can be pre-booked up to 4 -6
weeks in advance; the remaining can be booked from 8am the same day. Appointments can also be booked with
the Doctors or Nurse Practitioner online. In addition, patients are able to cancel an appointment online via the
practice website.
NAMED GP
As from 1st June 2015, all registered patients will have a named accountable GP recorded on their medical
notes. This means one of our GP Partners - Dr Hameed, Dr Sree or Dr Mohammed will have been allocated to you as
your 'named GP'. However, please be aware that this does not mean you have to see this GP all of the time. You
are still free to see a Clinician of your choice. The named accountable GP will take lead responsibility for the
coordination of all services required and ensure they are delivered to each of their patients. Please contact the
Practice if you would like to know who your named GP is. We will try to accommodate patient preference as much as
possible.

Aged 75 & over? Did you know we offer a full free health check with the Practice Nurse to discuss ways you can
improve your health and reduce chances of developing a chronic disease? Ask at Reception for more details.
PRACTICE STAFF

Receptionists: Tricia (Office Manager), Sheila, Lorraine, Janet, Rachael and Catherine
Receptionists are your first point of contact with the practice. Please help them to help you by giving them any
information they require to enable your request to be processed appropriately.
Healthcare Assistant: Elaine Gates, NVQ Level3 Diploma in Health and Social Care and Phlebotomy Level 2.
The Health Care Assistants role includes phlebotomy, blood pressure, ECG taking, spirometry, dressings, weight
management, smoking cessation and Healthy Heart Checks.
Nurse Practitioner: Geraldine Nuttall, RGN,Diploma Health & Social Welfare, Nurse Prescribing, Clinical Skills, BSc
Can treat a range of conditions and can prescribe many medications. Also provide telephone advices. Please be
aware that Nurse Practitioners are unable to see children under 12 months and pregnant ladies.
Nurses: Debbie Perkins, Nicola Vaughan & Delia Geldart, All Registered Nurses (RGN qualified) offer advice on
minor problems and who run many of the clinics mentioned in this booklet.
District Nurses: There is a team of nurses working in the community to care for the medical needs of housebound
patients.
Health Visitor: They are based at Low Grange Health Village and can be contacted on 01642 838003
Physiotherapist: Patients can be referred for physiotherapy through their GP or Nurse Practitioner.
