Patient Information Leaflet
To make our travel clinic service as safe and responsive for our patients and our clinical
team, we have produced this leaflet for your information.
Please try to plan your travel needs as far in advance as possible.
To enable us to plan your needs, we require you to complete the Foreign Travel Patient
Questionnaire (included in this pack). This allows us to tailor what is required for you and
give you a total cost where applicable and to order the appropriate vaccines.
Also included in this pack is a leaflet explaining the risks and side effects of each vaccine and
medication so that you are fully aware of these before you see us.
Once you have completed and returned your Questionnaire, one of the clinical team will call
you at a pre-booked time to discuss with you your needs and answer any questions you
have. If you agree to proceed, you will then need to book your appointment for our travel
clinic for any relevant injections and/or prescriptions. Please note that multiple
appointments may be required for certain vaccinations but you will be notified of this during
your telephone consultation.
To allow us to prepare for this, we will need you to agree to pay the full amount for your
travel needs at the time of booking this appointment. We can, of course, treat a whole
family in one slightly longer appointment (this will be discussed during your initial telephone
conversation).
Please note that payment must be made when you attend for your vaccination and that we
can only accept CASH or CHEQUE.
In rare cases, we MAY be able to fast track this process, should we have both the
appointment availability and the vaccines in stock (we do not normally keep a large stock
due to the costs). Therefore it is vital that you plan well in advance.
