SMART Goal Setting for Student Progress Form Teacher’s Name: Resource Teacher for the Gifted (SAMPLE-MATH) Subject/Grade: ____________________________________ School Year: ____ -____ Directions: This form is a tool to assist teachers in setting a SMART goal that results in measurable learner progress. NOTE: When applicable, learner achievement/progress should be the focus of the goal. Enter information electronically into the cells (the boxes will expand to fit the text). Initial Goal Submission (due by _____________ to the evaluator) I. Setting (Describe the population and special learning circumstances) II. Content/Subject/Field Area (The area/topic addressed based on learner achievement, data analysis, or observational data) III. Baseline Data (What is shown by the current data?) Incredible Elementary School has a population of 525 students of which 10% are identified for Gifted Services. This is a pre-selected pull-out math group of identified gifted students that meets once per week. Algebraic Thinking/Patterns unit using Project M3: Mentoring Mathematical Minds “Awesome Algebra” materials Pre-Assessment for M3 Awesome Algebra unit (see attached): Students will solve problems involving geometric patterns, numerical patterns, repeating patterns, growing patterns, 100-chart patterns, and mental problem-solving strategies. Pre-assessment will be scored using M3 unit test rubric (see attached). The assessment will be scored and results will be included in this baseline data section. Sample Data: see attached chart on last page Description: 0 out of 12 students scored 86% or higher 6 out of 12 students scored 80-85% 4 out of 12 students scored 70-79% 2 out of 12 students scored 69% and below Data attached IV. SMART Goal (Describe what you want learners/program to accomplish) During the ____ week unit, 100% of the second grade identified math students will show measureable improvement in algebraic thinking skills, as evidenced by an increase from the pre-assessment and postassessment scores. By the end of the unit, at least 85% of these students will score 90% or higher on the postassessment. V.
