General description form for assessment instruments In order to have a comprehensive general description of the assessment instruments, please adhere to this format. (Fill in table at the end of the document, using the following instructions) Name of the instrument: give the name of the instrument and if more versions exist, specify which version will be submitted Authors: give the names of all authors who created the instrument – in case the submitted instrument is a translation, also provide the language and the names of the authors of the original instrument Year of publication: give the year the submitted instrument was first published Type of assessment: e.g. formative or summative assessment – real patient or simulated patient interaction with health care provider – direct observation Topic covered: e.g. whole consultation or parts of consultation – verbal communication – non-verbal communication – dealing with emotions – shared decision making – breaking bad news – other specific focus Target group: e.g. undergraduate students or postgraduate trainees or residents – discipline (dentistry, nursing, medicine, physiotherapy, etc.) – other specific target group Assessors: e.g. faculty members, peers, (simulated) patients, … Languages: Describe what language the actual instrument you are submitting is in – we welcome instruments in all languages.
