29.06.2015 Lambeth Walk Group Practice PPG Meeting Minutes Date: 29.06.2015 Time: 6.30pm to 7.30pm Location: Lecture Theatre @ Lambeth Walk Group Practice LWGP Team attendees: Dr Rajive Mitra – GP Sarah Cross – LWGP Practice Manager New PPG Representatives: Matthew Dickens Dorothy Durant Thelston Durant Victoria Ajani Richard Craig 33 patients signed in at the meeting All patients invited via text message to the meeting Information was also displayed on the website This is the first meeting that was led by our new PPG Representatives (PPGR) 1. Introduction and run through agenda Sarah Cross – Practice Manager introduced the PPGR members and discussed the background to the PPGR – the vision for the PPG is to be for and represented by our patients. We have at several of the previous meetings and in the minutes discussed volunteers for this group. The first PPGR was held in March of this year and have met a further three times since. 2. Introduction from Matthew Dickens PPGR member. This also included further questions from the group as a whole. Matthew discussed the purpose of the group, the current group numbers of 5 patients with room for a further two. We have now had another volunteer, Yvonne Craig, so have room for one more volunteer. To contact Sarah Cross Practice Manager if interested in joining. We have created a terms of reference which is available on the website or on the PPG board in the practice reception. The purpose of the PPRG is that it provides a varied and patient led approach. Discussed practice demographics, prevalent illness. Discussed practical issue such as; having a water cooler in reception, hearing loops in the reception and the consulting rooms and looking at ways to improve the waiting area generally. 1 29.06.2015 Consider how we communicate with our diverse group of patients by age and background. Suggestion by Dr Mitra to contact schools so we can reach out to younger people. We do provide interpreter services here for patients. There was a query as to how the home visits are allocated. Dr Mitra explained that is it based on clinical need and only if a patient is unable to come to the practice for a clinical reason e.g. is clinically housebound. 3. Sarah Cross - Practice Manager discussed the friends and family test and repeat prescriptions. Brief explanation of friends and family – a new service whereby patients can give feedback at every visit either by completing a paper format at reception or going to the https://www.iwantgreatcare.org/ website and typing in the practice name. The purpose of this feedback is to gain regular, up to date feedback and be able to act on current issues as and when the feedback comes in. Repeat prescriptions – the practice is aiming to have all patients on the electronic prescription service by October 2015. This means that a patient requests a prescription then this is automatically sent electronically to the nominated pharmacy of the patient’s choice. There have been some issues with this which the practice is currently working on with the pharmacies in order to ensure that the process is smooth. Further information will be provided to patients regarding this over the next couple of months. Discussed the relatively new online access service. Patients can book appointments, request prescriptions and access some patient information via https://patient.emisaccess.co.uk/Account/Login. To register you need to visit the practice and complete the online access consent form and provide the practice with photographic ID. This is then given to a team member to process and confirm. This process is in order to ensure that access is only given to the patient. For access to children’s files by a parent, guardian or other files by a person who has medical power of attorney this is managed on a case by case basis. 4. General question and feedback Can we create an online forum? Sarah Cross will look in to how we could create this on the website. Out of area – if you move out of area in certain circumstances you can remain at the practice – for further information please contact the reception team by visiting, emailing lambethwalkgp@nhs.net or calling on 0207 735 4412. General consensus is that the access to the clinical team is very good – all patients can be seen or spoken to on the day. Although sometimes there is a wait especially for the walk-in with, resources as they are this was generally felt to be a very good service. To provide a feedback box in reception specifically for PPG ideas. Consider getting Wi-Fi – the practice would have to consider the cost for this. 2 29.06.2015 Discussed having what the drugs are for added to the patient information sheet that comes with the dosset boxes e.g. high blood pressure tablet, pain relief etc. Sarah Cross to discuss further with Dr Mitra as to how this could be implemented. Discussion around patients loudly using their mobile phones in the practice waiting area. To consider how we can manage this. Discussed risks in the practice and the use of checklists in hospitals (based on the Aviation Authorities model) to reflect their culture of openness to errors so that they can learn from them. Dr Mitra discussed the policy of openness within the practice and how we evaluate and discuss any issues that arise in order to learn from them and ensure they don’t happen again. Next meeting planned for September 2015 – date to be confirmed. 3
