CHAPTER ANNUAL REPORT 2015-2016

Chapters should update this document throughout the year in order to track progress, goals, and
accomplishments. Chapters will be expected to submit this document to their council advisors
by December 11, 2015 as the Mid-Year Report and then by May 10, 2016 as the Annual Report.
Mid-Year Report: Chapters should respond to the questions in Section I: Programming Areas for
each of categories that the chapters completed programming for during the fall semester. We
encourage you to fill out service hours and philanthropy events from the fall semester at this time
as well. The more information that chapters can provide in the Mid-Year Report, the less work it
will be at the end of the spring semester to complete the Annual Report.
Annual Report: All questions should be answered and all requested information should be
provided in this document.
