APPENDIX A STATEMENT OF WORK 1. Scope of Work/ Purpose The selected firm will provide a scope of work that will show how they intend to meet the County’s need for American Heart Association (AHA) cardiac life support training for healthcare providers throughout Santa Clara Valley Medical Center (SCVMC) and Ambulatory and Community Health Services (ACHS) of the Santa Clara Valley Health and Hospital System (SCVHHS). The selected firm will be expected to be able to teach the following types of cardiac life support training courses to our healthcare providers upon request: • • • • • Basic Life Support (BLS) Adult Advanced Cardiac Life Support (ACLS) Optional Basic Life Support Skills Checks in conjunction with ACLS recertification Pediatric Emergency Assessment Recognition & Stabilization (PEARS) Pediatric Advanced Life Support (PALS) The selected firm will be able to teach cardiac life support training courses (initial certification and re-certification courses) sufficient in number to accommodate the County’s needs. The annual number of employees that may participate in BLS, ACLS PEARS, and PALS Certification and Re-Certification courses are estimated as follows: Course Type Recertification Certification BLS 900 180 BLS Skills Checks 150 N/A ACLS 300 75 PEARS 30 PALS 150 50 The courses will be evidenced-based and will include both didactic review and skill performance based on mock cardiac arrest scenarios and proper compliance with cardiac life support matrices based on lab results, electrocardiogram (EKG) readings, and patient parameters such as vital signs. Specific Requirements for Service Delivery SCVMC: 2.1 The selected firm will be required to teach a series of SCVMC BLS and PALS certification and recertification courses on the SCVMC Campus in classroom space provided by the Nursing Educational Services Department, located at 2408 Clove Drive, San Jose, CA 95128. These courses will be held on specific dates that will be provided and scheduled by SCVMC. 2.2 The BLS courses will be offered on only 1-2 dates per month. The Nursing Educational Services Department typically utilizes 3 separate BLS classrooms with 3-4 instructors per classroom, depending on class size (instructor to student ratio is 1:4). The maximum number of students that can be accommodated in the 3 classrooms on each course date is 40 students. These courses will be offered in AM and PM sessions, accommodating a maximum of 80 students per day. 2.3 The PALS courses will be offered on only 1 date per month during 9 months each year. There are approximately 25 students in each of the 9 courses. There are 7 PALS recertification courses and 2 PALS certification courses offered each year. The Nursing Educational Services Department utilizes 3 classrooms for each of the PALS courses. ACHS: 2.4 The selected firm will be required to teach a series of BLS recertification courses for ACHS utilizing a conference room adjacent to the SCVMC Campus at 770 Bascom Avenue, San Jose, CA 95128. Approximately 15-20 of these courses will be offered each year, with class sizes ranging from 10-20 for each course offered. Courses will also be requested at other ACHS locations such as the Gilroy Clinic, the Tully Clinic and the East Valley Clinic. It is estimated that 4 of these courses will be offered each year, with class sizes ranging from 15-20 students for each course offered. Monthly schedules of course dates and times will be provided by ACHS. 3. General Requirements For Service Delivery (ACHS and SCVMC) 3.1 The selected firm will also be required to teach a sufficient number of ACLS certification and recertification courses, primarily for students from SCVMC, and PEARS certification and recertification courses primarily for students from ACHS to meet the number of estimated students referenced in table above, Item one, Scope of Work/Purpose. The training/course location will be provided by the firm. Page 2 of 4 3.2 ACLS, and PEARS training for healthcare providers will be determined by the potential vendor’s monthly schedule of ACLS and PEARS courses. The schedule must include the types of courses and the dates and times courses that will be offered. The schedule of courses must be provided months in advance to schedule trainings. 3.3 The selected firm will be expected to provide customer service to the County from 8:00 am to 5:00pm weekdays and will be expected to provide an annual schedule of days that will be considered holidays with their proposal and at the beginning of each calendar year thereafter. 3.4 The selected firm will be expected to provide the following customer service functions during business hours, 8AM to 5PM, Monday - Friday : Scheduling/registering, confirming, cancelling, or rescheduling our healthcare providers for cardiac life support training courses. Answering questions and receiving inquiries related to billing, invoicing, contract and insurance compliance, and other business matters. 3.5 The selected firm will be required to provide all equipment necessary to teach the required courses. 3.6 The selected firm must be an authorized American Heart Association Emergency Cardiovascular Care Training Center. 4. EXPECTED OUTCOMES 4.1 The healthcare provider is provided with the information and performance skills necessary to successfully perform either: BLS, ACLS PEARS, and PALS. 4.2 The healthcare provider receives a passing score on both written examination and skill performance. 4.3 The healthcare provider is either re-certified or certified as proficient in either BLS, ACLS PEARS, or PALS and receives evidence of course completion and a certification card to be carried with them at all times while performing their duties as a healthcare provider. 4.4 The healthcare provider is provided continuing education units (CEUs) that are commensurate with the number of hours required for each course that is successfully completed. GEOGRAPHICAL/REGIONAL SERVICE AREA The selected firm will be required to provide training in Santa Clara County as specified under Sections Two(2), Specific Requirements for Service Delivery, and Sections Three (3), General Requirements for Service Delivery. The County’s preference is that the nonCounty designated training facilities used by the selected firm for provision of services be conveniently located near the SCVMC Campus, located at 751 S. Bascom Avenue, San Jose, CA 95128, for the convenience of SCVMC and ACHS healthcare providers and to minimize travel time to and from the course location. 6. DATA COLLECTION AND REPORTING REQUIREMENTS With each monthly invoice, the selected firm will be required to provide course participant lists to SCVMC and ACHS for each type of cardiac life support training course that is offered that includes the following: The name of the healthcare provider The department of the healthcare provider at SCVMC or ACHS The type of course scheduled to be taken The course completion status (i.e., course completed, no show, etc.) 7. PAYMENT/INVOICE 7.1 The selected firm will be paid according to negotiated and agreed upon course description fees proposed in the Proposal Cost Response Form, Appendix B. 7.2 The selected firm will be expected to provide separate and distinct monthly invoices for SCVMC and ACHS. 8. ADMINISTRATIVE PROCEDURES The selected firm will be expected to provide a cancellation policy for course cancellations with their proposal. 9. HOURS General business hours i.e. 7AM- 6PM and specifically, 8AM-5PM for Customer Service, Monday - Friday. Page 4 of 4
