Information Packet and Checklist
PLEASE:
 Read this entire packet
 Make a family decision that participation in The Nutcracker will fit
into your schedule.
 Fill out and return the REGISTRATION FORM along with the $15.00
Placement fee by Wednesday, September 1st.
 While participation in The Nutcracker is an exciting, educational
and fun experience for the children, please do not forget that it is
also a very serious commitment for the entire family.
If you have read this entire packet and still have questions call Angie
Neace at (352) 637-4663 during regular business hours.
IMPORTANT INFORMATION
Please consider the following conditions carefully before making the family
decision to allow your child to audition, as you automatically agree and commit
to them if you choose to attend:
 Faithful and punctual attendance at rehearsals and performance is required.
Please note that it is vital for the entire cast to attend each and every
rehearsal without fail. As there is a lot of material to be learned, your
fullest support and corporation is crucial to the success of the production.
 Rehearsals will begin the week of September 1st. See the attached tentative
Schedule. However students 8 & up MUST attend the “Nutcracker” placement day
scheduled for August 22nd.and MUST attend the “Nutcracker” workshop week August
22 – 26, 2011.
 Your child’s participation in The Nutcracker cannot be linked to transportation
provided by another cast member.
 We do not offer complimentary tickets or passes to the performance.
 Performing children will be monitored backstage by NUTCRACKER staff
and selected volunteers.
 Participation fee, payable by October 1st, is non refundable. This goes
towards the cost of rehearsals, costume fitting and maintenance.
ROLES AND CASTING ARE ASSIGNED BY THE ARTISTIC
DIRECTORS.
ALL DECISIONS ARE FINAL.
Shoes, tights, leotards and make-up are to be provided by the
student.
PLEASE NOTE: NO VIDEOTAPING OF ANY REHEARSAL OR PERFORMANCE
WILL BE PERMITTED.
THE PLACEMENT DAY
 Bring the completed, signed PLACEMENT FORM, the $15.00 placement fee
and the LIABILITY & MEDIA RELEASE FORMS.
 Students should come to audition dressed to dance – Leotard, Tights,
Ballet Slippers.
 At the end of the placement process, for each age division, the students
will be gathered together in one studio. Only after the children are in the
studio will the parents be invited back to claim your child. We will hold
the children in the studio until a parent or responsible adult comes
to claim them.
Remember, by signing the PLACEMENT FORM, you (and your child)
have agreed to accept the decision of the Artistic Directors of
Step ‘N Time School of Dance Arts regarding role, cast, costume and
performances. Every effort will be made to cast as many
students as possible, however this includes the unfortunate
possibility that your child may not be selected.

Hold Harmless Agreement
I, the undersigned parent/guardian, give my full permission for my child to register and
participate in the rehearsals, coaching sessions and production of The Nutcracker 2011
and hereby release and hold harmless Step ‘N Time School Of Dance Arts, it’s affiliates,
assignees, contractors, lessees and personnel from any liabilities and claims sustained
by my child while participating in the 2011 production, rehearsals and coaching sessions.
Permission to Film/Photograph
In a production such as The Nutcracker, it is likely that photography, live transmission
and/or video will be taken of your child. By signing below, you are authorizing the
filming and photographing of your child and understand that there is no
compensation to you or your child for such.
Behavior Agreement
Proper and good behavior is an absolute must to participate in a production such as
this, which requires strict discipline. Participants are expected to behave in a responsible
manner throughout the rehearsals and performance. It is required of each participant
to show respect at all times for others and the property we use. This also includes
being on time, staying on task, following directions the first time, paying attention
and self-control when you are working and when others are working. Your signature
below indicates that you have read and understand this agreement and agree to
abide by it. Any non-compliance to this agreement after it is signed can mean
immediate dismissal from the production and forfeiture of fees paid.

SDA and its staff the right to obtain medical services for the signature below. I also understand that fully receiving good dance
training may involve adjustment of the dancer’s body by the treacher.

Please read through the following agreement, initial each statement, sign the bottom and return to Angie Neace.
_______I understand that a non-refundable check for $100.00 per child, made payable to School of Dance Arts will be due
October 15, 2011.
_______I understand that my child must be present for all of his/her rehearsals* and that casting for “The Nutcracker” is the
decision of the Directors of School of Dance Arts. The School of Dance Arts will not enter into any form of discussion with
students, parents or patrons relating to the casting of “The Nutcracker 2011”. All decisions made by the Directors are final.
*Exceptions will be made for illness or family emergencies only.
______I understand that my child must sell ten (10) tickets for the “The Nutcracker 2011”. This is a mandatory contribution to the
production of “The Nutcracker 2011”.
______I understand that all costumes for “The Nutcracker 2011” will remain the sole property of The School of Dance Arts.
______ I understand that I am responsible for purchasing the proper tights and shoes for my child.
______I understand that should I withdraw my child from “The Nutcracker 2011, he/she will not be able to re-enter into the
production in 2011. All monies paid to the production are non-refundable.
_____I understand that if my dancer(s) receives a solo in “The Nutcracker 2011”. He/she will receive one (1) private coaching
session free of charge. Any additional coaching sessions(s) if necessary will be charged at a rate of $25.00 for a 45 minute session

____ I understand I must commit to help in the following ways to raise funds for the production of “The Nutcracker 2011”.
1.
Sale of Advertisement for the program. To value $75.00 or more.
2.
Yankee Candle Fundraiser. Each person is required to sell a minimum of $50.00

Parents choosing this will make a personal donation of $150.00 per participating Child, to “The Nutcracker 2011” directly which
releases you of any further obligation to partake in the fundraising activities.
