Nutcracker Field Trip
On Wednesday, Nov. 28, our students will be attending a field trip to a performance of
the Nutcracker at the Capitol Theatre in Salt Lake City. We will be riding a bus to the
theatre.
We will be leaving the school at 9:30 A.M. and returning about 3:00 P.M. Because we
will be arriving after school is out, you will need to make arrangements to pick up your
student when we return.
ALL STUDENTS WILL NEED TO HAVE A SACK LUNCH. You can either bring a
lunch from home or order one through the school’s lunch room. (Talk to Mrs. Gray by
Tuesday, Nov. 27, if you need to order a lunch.)
If you consent to your child participating in the event, please read and sign the following:
1. I give my permission to have my child participate in this event.
2. I understand that if any injury occurs the school will make reasonable efforts
to contact me. In the meantime, I give my express consent, in the event of
injury, that my child receives emergency medical aid, anesthesia and/or
operation, if in the opinion of the attending physician such treatment is
medically necessary.
3. I have made the necessary arrangements to pick up my student when they
arrive back at the school.
