The Miami-Marquette Challenge 2013-2014 Course Course Title: “I tore my ACL - what should I do? When can I play? Do I need surgery?” – Applying the Available Evidence to Counsel Patients and Optimize Outcome with Consideration of Patient Preferences Course Speaker: Lynn SnyderMackler, PT, ScD, FAPTA Date: Saturday April 12, 2014 Time: 8:00 am-1:15 pm Registration begins at 7:30 am Location: Centers for Rehab ServicesSports Medicine South Side Pittsburgh, PA Direct Access CEU’s: 0 General Access CEU’s: 5 Total CEU’s: 5 Cost of Course: $100 All proceeds from this course will be donated to the Foundation for Physical Therapy as part of the MiamiMarquette Challenge. Registration online: http://pittphysicaltherapy.com/ Registration by mail: (see last page) Course Objective: After completing this course, participants will be able to: 1. Counsel individuals with an ACL injury on their options for both surgical and non-surgical management and educate them on their plan of care. 2. Implement neuromuscular retraining techniques (perturbation training) to normalize and improve lower extremity stabilization strategies to improve the chance for success with non-operative management. 3. Identify appropriate treatment techniques to improve quadriceps strength both before and after ACL reconstruction that are effective and safe. 4. Counsel individuals after an ACL reconstruction on their potential for re-injury after with a return to cutting and pivoting sports. 5. Use evidence based functional tests to determine readiness for return to sports after operative and nonoperative management. Course Description: Anterior cruciate ligament injury presents a significant barrier to participation for an athlete, impacting the current competitive season and potentially subsequent competitive seasons. Return to sport rates after ACL reconstruction are poor and there is no clear advantage to reconstruction over non-operative management. In situations where no treatment is clearly superior, treatment decisions should be made based on the preference of patients after being properly educated on their likely outcomes with different treatment options. Similarly, returning to sports after ACL reconstruction is associated with a greater injury risk than for individuals who have never been injured, thus making education and minimizing injury risk crucial in optimizing outcome. This course will summarize the available data to facilitate patient education for decision making concerning operative versus nonoperative management and the risk of returning to sport after ACL reconstruction. Treatment techniques to improve the potential for success will also be included both didactically and in laboratory sessions, including perturbation training to improve dynamic knee joint stability, progressive quadriceps strengthening, and functional testing to evaluate performance for informed decision making. Over the past 25 years, the work of Lynn Snyder-Mackler and her laboratory at the University of Delaware has sought to identify clear and simple interventions to improve the outcomes for patients after ACL injuries. This has included collaborations across the country and in Europe, the results of which will be highlighted. Course Schedule: 8:00-8:30 –Post-injury and PreSurgery Rehabilitation 8:30-9:45 –Non-operative rehabilitation to focus on advanced neuromuscular re-education return to high level activities and adaptation to less demanding activities 9:45-11:00—Demonstration and Lab Session for Perturbation Training Techniques 11:00-11:15- Break 11:15-12:15- Progressive quadriceps training exercise including neuromuscular electrical stimulation 12:15-1:15—Late phase rehabilitation to return to activity/Demonstration and Lab Session for NMES and Hop Testing About the Speaker: Dr. Snyder-Mackler is an internationally recognized clinician and clinical researcher in sports and orthopedic rehabilitation. She is a Board Certified Sports Physical Therapist who maintains an active Sports Physical Therapy practice at the University of Delaware and serves as a rehabilitation consultant to collegiate, amateur and professional teams. She served as Head Trainer for the beach volleyball venue at the 1996 (Centennial) Olympic Games in Atlanta. She concentrates her clinical practice and research in the areas of knee and shoulder rehabilitation, and electrical stimulation of muscle. She has authored more than 150 research publications in the areas of knee and shoulder rehabilitation and neuromuscular electrical stimulation and regularly speaks to national and international audiences on these topics. She was named a Catherine Worthingham Fellow of the American Physical Therapy Association in 2003 and Alison Professor at University of Delaware in 2010. Her research has won several major awards helping patients and practitioners and answering critical questions in sports and orthopedic rehabilitation.
