There has been an increased focus on the comparison of different femoral tunnel
drilling techniques in ACL reconstruction to attain maximum knee stability, and
clinical outcomes. However, few clinical studies report the outcomes of these
methods, especially in terms of sporting ability. We aim to compare the clinical
outcomes and ability to return to sports between both transtibial and transportal
drilling methods.
A retrospective review of the records of 46 patients (25 transtibial; 21 transportal)
undergoing primary ACL repair in our institution from 2009-2012 was performed.
The average follow up was 36 months. The clinical outcomes of both groups of
patients were assessed using the Lysholm and Koos knee scoring systems.
Ability to return to sports were assessed using the Tegner Activity Level Scale.
Our patients generally had a marked improvement in clinical symptoms post ACL
reconstruction; with an average Koos score of 97.1 (73 - 100) and Lysholm score
of 96.5 (62-100). The average time to return to sporting activity was 9.6months
(0-48) while the average time spent undergoing postoperative physiotherapy was
6.2months (1-24). Comparing the scores between the transtibial and transportal
groups, there were no significant differences in knee scores and return to sports
levels; with an average Koos score of 96.6 versus 97.7 respectively and an
average Lysholm score of 95.8 versus 97.3. The average pre-injury Tegner level
was 6.9 versus 7.5 and post injury Tegner level was 6.2 versus 6.6. A greater
drop of Tegner levels in the transportal group was observed, but was not
significant on analysis (P>0.05).
In conclusion, our study showed that there are no significant differences in
clinical outcomes between patients undergoing the transportal and transtibial
methods of ACL reconstruction, including the time to return to sport, Tegner
activity levels and functional knee scores.
