Reporting Process for:
A clinical incident or near miss that has directly impacted upon a student
Specification:
The following flowchart illustrates the process to be followed when a student is involved in a Clinical
Incident or Near Miss.
This process may trigger further University of Derby and host organisation processs and
interventions at different stages of the process that are not specifcally detailed in the flow chart (for
example, although not exclusively: Professional Conduct and Professional Suitability; Raising
Concerns).
*No patient/service user information should be disclosed in this specific University incident/near
miss reporting process, it should refer to the actual or potential impact upon the student/s only.
Student involved in a Clinical
Incident or Near Miss during a
placement experience that (or
had the potential) impacted
upon them (in the form of
personal harm or injury).
Host organisation reporting
system is separately followed
by the student mentor or
supervisor
Student is reminded by
mentor/supervisor of the student
requirement to complete the
