AED National Membership Application 2015
Please review the following requirements for national membership before completing the application. This
page must be signed and stapled on top of your application. Applicants who do not meet these requirements
will not be considered for membership.
General Requirements:

The student must be taking courses leading to a career in the health professions.

Character, general ability, and personality must be considered carefully in the selection of every
member.

Members shall be elected without any discrimination on the basis of race, sex, religion, creed or
national origin.

The student shall be an associate member of AED when applying for active national membership.
Specific Requirements:

Must be a sophomore or above.

Must have been a pre-health student for at least 3 semesters and must have been attending Auburn
University for at least one semester.

Must have at least a 3.2 cumulative, ungapped GPA and a 3.2 cumulative, ungapped BCPM GPA
(Biology, Chemistry, Physics, Math).

Must have a record of attending at least 7 AED Meetings.

Must have acquired at least 4 AED Service Points.
Requirements for Retaining National Membership after Induction:

Must have a record of attending at least 4 AED Meetings per academic year.

Must acquire at least 3 AED Service Points per academic year.

Due: Tuesday, February 10, 2015, by 4:00 p.m. in COSAM Advising Office, SCC 239
** You may also bring it to the AED Meeting on February 10th.
Notification: Friday, February 15, 2014 by 5:00 pm via email
Spring Initiation: Tuesday, April 7, 2015, at 6:00 p.m. in SCA
Instructions: Print and fill out all 5 pages of this application, including the requirements signature page (page
1) stapled on top. Bring completed application to the COSAM Advising Office (SCC Room 239) and place it in
Mrs. Childress’s mailbox. Applications are DUE by 4:00 pm on Tuesday, February 10, 2015. Please type or
write legibly, and attach a 2x2” photograph in the designated area. Please do not worry about having a
professional photograph. All applicants will be notified of whether or not they are granted membership on
Friday, February 15, 2014 by 5:00 pm via email. Send any questions to Lindsey Schapker, AED President, at
lms0025@auburn.edu. If you need more space for your answers, please feel free to attach any additional
