Sullivan VP, Operations & Development Thomas S. Rimel, Jr. I. SUBJECT: Tuition Reimbursement II. PURPOSE: To provide assistance to all full-time associates in continuing education programs. III. POLICY: DUNMORE will provide tuition assistance to full-time associates who have completed 90 days of employment and who desire to further their education. Tuition assistance will be provided for courses taken at accredited institutions (colleges, universities, technical schools, or high schools), for the purpose of obtaining a degree (MBA, BS, AS), certification, accreditation or diploma (GED). The course or curriculum must be considered beneficial to the Company. IV. PROCEDURE AND RESPONSIBILITY: Before registering with an institution and before starting classes, associates are encouraged to ensure their degree program will be approved and considered beneficial to the Company. The executive of the associate, with HR, will pre-approve the degree program. Prior to each semester’s registration, enrolling associates must complete an application form and submit it, with a copy of the course description and tuition cost, to their supervisor for approval. The supervisor’s signature of approval is stating that the associate is in good standing and is not on any corrective action steps for performance or attendance. The application is then forwarded to the Human Resources for review of eligibility, and processing. Human Resources will determine eligibility of the associate and the course, forward to the associate a signed authorization of approval, submit requests for payment, and maintain records. A maximum of 2 courses will be approved at any given time, with a maximum annual expense not to exceed $7,500. Reimbursement will be limited to course fees, textbooks, and lab fees. Note that any reimbursement that exceeds the IRS maximum will be included as taxable wages to the associate. Reimbursement will be granted after the associate has submitted a copy of the official grade(s) to Human Resources, together with an official payment receipt for the cost of the tuition, provided DUNMORE CORPORATION Corporate Policy and Procedure Tuition Reimbursement Page 2 of 3 the associate is still an active employee at the time of payment. The Company also offers an advance for tuition (see below). Reimbursement of 100% is contingent upon attaining a grade of C or better, or a mark of satisfactory if a grade is not used. Grades of less than a C, or unsatisfactory, or incomplete, or a withdrawal, will not be reimbursed. DUNMORE is committed to assisting our associates in their educational objectives; therefore, we will advance the tuition as follows: DUNMORE will advance 100% of the tuition for the first class(es). This advance will be recorded as an IOU from the associate to DUNMORE until the class(es) is completed. Upon submission of the grades, HR will process a reimbursement check for the full amount of tuition, books and fees. That reimbursement is then to be used to fund the next class(es). Each semester an accounting of the actual expenses will be computed. If the associate fails to complete a course or terminates employment, the original advance (IOU) must be repaid to DUNMORE. If the associate completes the program, graduates, or elects not to continue with any additional courses, the reimbursement for the last course taken will revert back to DUNMORE to settle the outstanding advance (IOU). If the associate is not entitled to reimbursement because of failure to complete the course with the required ‘C’ grade, the associate must pay back any advances (IOU) to DUNMORE within 60 days via personal check or through payroll deduction coordinated by Human Resources. The associate will not be entitled to any future advances until full repayment to DUNMORE has been made. Associates voluntarily (resignation) leaving DUNMORE, or involuntarily (just cause) within twenty-four (24) months of having received an advance, or having been reimbursed for any course(s), will reimburse DUNMORE for any and all advances / reimbursements for tuition payments made to them in that period based on the following schedule. 100% repayment for all courses reimbursed in the 12 months prior to separation 75% repayment for all courses reimbursed between 12 and 18 months prior to separation 50% repayment for all courses reimbursed between 18 and 24 months prior to separation 0% repayment for all courses reimbursed 24 months or longer prior to separation The associate may be asked to provide an evaluation of the course detailing the value gained from their attendance. They must also be prepared to make a presentation to their colleagues if their supervisor / manager believe the subject matter would be of value to others. The Company may make accommodations to adjust work schedules to allow associates to attend classes. Business necessity will determine decision. Prior to registration the associate completes this form and submits it, together with a description of the course(s) and tuition cost to their supervisor for signature approval. 2. Associate then submits the application to Human Resources who will then return a fully signed copy of the application indicating approval of course(s) for the semester. 3. After you have completed the course(s), submit a copy of the official grades and tuition / book receipts to Human Resources.
