Deadline for Submission: April 24, 2015, to Jane Rock, Mississippi Bend AEA, 729 – 21st Street, Bettendorf, IA 52722

On target for high school graduation and 90%
attendance or better in most recent semester of
high school
Completion of your district’s web-based career
research and/or planning requirements for junior
and senior year (Either IHaveAPlanIowa or Career
Cruising)
COMPASS or other college placement assessment
Post-Secondary Course of Study in place
Artifact or Document Required
(to be retained by student)
Letter and/or transcript from your high school; if
attendance less than 90%, an explanation from high school
about why this was not possible.
Copy of each of the six profiler/survey components on
www.ihaveaplaniowa.gov preferably from last 2 years
Placement scores or explanation if not needed for selected
post-secondary program
(1) Proof of application or evidence of enrollment
(2) Outline of program requirements
(3) Student’s action plan, listing steps to complete for
post-secondary enrollment and success
History of successful participation in one or more
of the following:
 Employment
 Work Experience (paid or unpaid)
 Two Job Shadows
One of the following:
 Written reference from employer
 Signature from Work Experience Service Provider
 Examples of work evaluations
 Job Shadow information/materials
Ability to describe strengths and any
accommodations helpful for learning or working
environments (SAR could be used)
Signature of school or T.A.B. official (on this Checklist)
Up-to-date resume and Letter of Recommendation
from School or Employer
Resume online or in print, plus copy of letter of
recommendation supporting career choice, post-sec plans
Attend at least one:
Signature verifying attendance by one of the organizers of
the event or high school official or parent




