ANESTHESIA FOR IVC FILTER REMOVAL Anesthetic choice: -MAC -Rarely GA (can be a long case) Position: -Supine -Both arms tucked -Table turned 90 degrees(L side of patient toward us) Lines: -1 IV( to L hand if possible, easier for us) -No arterial line Pharmacology: -Can use sedation of choice(Versed, Fentanyl,Propofol) -Need patient awake after Foley (if used) and wires in(want patient to hold breath during cavagram for quality pix) -May use Heparin(have available) Tips: -Surgeon will access from Right IJ(Filter can only be removed that way because of its shape) -Cavagram will be done before filter removal to be sure no clots in basket -Alignment of removal grasper and filter attachment may be surgically difficult leading to lengthy case.
