Please rate your UTSA student Junior Achievement volunteer so that I can give an oral presentation grade to the
student. Please be very critical in your evaluation—this is designed to help the student know his/her
presentation strengths and weaknesses. (As a benefit for participating in this program, the student gets full
credit for the final examination.) Please complete only one of these forms after all of the student's JA

By April 27 (or earlier), please mail, scan and email, or fax this completed evaluation to me (see contact info
below). I am unable to accept a completed evaluation directly from the student without it being in a
