Signature of Supervisor: __________________________________ Date: _____________ CARDIOTHORACIC SURGICAL LOG BOOK SUMMARY This logbook summary is a record of operative statistics compiled during Specialist Training in Cardiothoracic Surgery. Operative procedures performed during the General Surgical component of training should be recorded in the format used for General Surgery trainees. Beginning in January 2003, the guide to Specialist Cardiothoracic Training prescribes minimum numbers and case-mix that needs to be achieved in a given year of Specialist training. The new logbook summary reflects those pre-requisites (in the case of thoracic surgical training those case figures are spread over a two year period). The Cardiothoracic Board believes that trainees should be adequately supervised for all operative procedures and this logbook recognises three levels of operative assistance: A. Operation or component skill performed BY TRAINEE without the surgeon in charge actually assisting with that operation or part of the procedure. (The surgeon will decide the appropriate oversight for that patient and trainee and may be in the operating room). The operative procedure or skill in question may be only part of the main procedure (e.g. cannulation for bypass or radial artery harvest). B. Trainee Assisted, by senior member e.g. Fellow or surgeon, to perform a particular procedure. C. Trainee is the First assistant for the procedure. Do not record procedures or operations where you are the 2 nd assistant. This operative list is comprehensive but will not include every conceivable cardiac procedure. Procedures not listed will be entered in the “OTHER” category. The trainee will present this summary of clinical experience, when advised, twice each year for review by the entire Board at its meetings. The trainee will need to keep all raw data regarding clinical experience in a form that would allow verification of the caseload and casemix at a later date if that should be required. The data should be kept in a fashion that is consistent with College guidelines regarding patient privacy (identifier by MRN only). This document must be signed before being submitted, by the trainee and the Supervisor of Cardiothoracic Surgical Training and as such indicates that this is an accurate record. Forms will not be reviewed if they are not signed by both parties.
