BLOOD ORDER GUIDELINES FOR SURGERY (BOGS)
PURPOSE
Describes UCLA's Blood Order Guidelines for Surgery (BOGS) and how it is used.
SCOPE/PRINCIPLE
 BOGS was developed by analyzing blood use in UCLA Operating Rooms over a 6-month period. Blood
order numbers were calculated to meet the needs of 80-90% of patients undergoing the corresponding
procedure and then modified to more realistically represent "standard blood orders". Type & Screens
are recommended for procedures that require less than 0.5 unit per patient per procedure.
 BOGS data is periodically reviewed by representatives from the Department of Surgery and Transfusion
Medicine and updated as needed.
 Surgeons or anesthesiologists may individualize or override BOGS to accommodate special
patient needs.

BOGS LIST
Review Date: May 16, 2006
TYPE & SCREEN (T/S) provides information about the patient’s blood type and the presence of red cell
antibodies.
- If the antibody screen is positive, additional work must be performed to identify the antibodies present
and to rule out other clinically significant antibodies to red cell antigens. This additional work may be
completed within a couple of hours, or may take days and require send-out to a Reference Lab.
- Completion time and blood availability depends on the number of antibodies present, antibody reactivity,
and frequency of the corresponding antigen in our donor population.
- Once clinically significant antibodies are identified, national standards require the Blood Bank to provide
antigen-negative, crossmatch-compatible units for transfusion.
TYPE & CROSS (T/C) is an order for a T/S plus a request for a specified number of RBC units.
- Depending on the results of the T/S, units may be readily available (no clinically significant antibodies
identified) or may require special screening for antigen-negative units (when clinically significant
antibodies are identified).
