Integrated Care Teams
• Three clusters involving all B&A practices
– North Bracknell

• Over 340 people have a multi-agency care
plan already
• More investment this year to increase mental
health, social work and community matron
capacity

Musculo-skeletal ICATs and physiotherapy
• Following the learning from a pilot service, the
contract for a new ICATs was tendered this year
• The contract was awarded to Healthshare
• New service started from Healthspace in July 2014
• Early signs are that more people are being helped to
resolve musculo-skeletal problems without the need
for surgery
• Community physiotherapy has been transformed and
a new contract awarded to BHFT
• A new knee clinic is also being trialled, in Ascot,
which offers people an alternative to knee
replacement surgery for those who would prefer this
