Whilst moving the Families and Social Care (Adults) Directorate to a position where, by 2015, it can operate on a reduced budget. 13 Our vision is: To support the social care needs of vulnerable adults in Kent, by ensuring that services are available which enable them to live fulfilling and independent lives in their homes and communities. We will do this by: •Promoting independence, wellbeing and choice for individuals •Promoting and supporting resilience and self care •Providing proactive and responsive support •Developing a market of services which are high quality and represent value for money 14 Our stakeholders have told us they want: •A system that is easier to understand and navigate •Comprehensive information, advice and guidance that is accessible •A proactive rather than reactive system •Integrated services designed to maintain a person’s lifestyle and engagement with their community. •The same service offer for self-funders and those in receipt of KCC funding •Integrated health and social care services – single points of access and less professional involved •Services that are responsive to local need •Targeted interventions that aim to restore a person back to a preceding state of health and well-being 15 Introducing Community Support Unit Emma Hanson Head of Strategic Commissioning Community Support Karen Cook Commissioning Manager Steve Lusk Commissioning Officer Sholeh Soleimanifar Commissioning Officer Sam Sheppard Commissioning Manager Heather Randle Commissioning Officer Laura Embery Commissioning Support Assistant Jo Empson Commissioning Manager Carina Neeves Commissioning Officer Tony Care Commissioning Officer Commissioning Support Assistant Sylvia Rolf Commissioning Officer Sarah Peacock Commissioning Officer JenniferNewton Commissioning Support Assistant Simone Bullen Commissioning Officer Commissioning Support Assistant Guy Offord Commissioning Officer Julie Harling Commissioning Support Assistant James Lampert Commissioning Manager Hazel Price Commissioning Officer Paula Parker Commissioning Manager David Harris Commissioning Officer Navdeep Mandair Commissioning Officer Commissioning Officer Vacancy Mick Jones Commissioning Officer Helen Devlin Commissioning Officer (Mat leave until March 2013) Commissioning Support Assistant TBC Sarah Challis Commissioning Officer (Mat Leave begins Nov 2012) Information in your packs Introducing Community Support Unit Supporting Rehabilitation and Recovery Restoring people to independence Help to stay at home - key strategic partners in community Develop Community Capacity Suport people remain independent of social and health care system Grants- a picture of spend We are spending £15 Million a year in grants • We are NOT looking to cut funding, in fact we want to increase spending on preventative services through the Health Money BUT we want to spend it on the right things: • When every penny counts are we making the best use of this money to keep people out of the health and social care system? Grants- a picture of services • Grants have grown organically and been locality driven • Different services in different areas- no core offer • You provide other excellent services we don’t fund • Grants not always strategically aligned to Transformation • Cant performance manage a grant • Instability of yearly funding • Unwieldy system and bureaucratic Questions: How do we ensure equality of access and provision across Kent? What should KCC fund and for whom? What services should we move from grant to contract? A Picture of Need 2012: 900 Kent Social Care Users of all ages responded to our annual survey: • 25% of service users said they did not have enough social contact. • 66% of these lived in the community 19% say information and advice is difficult to find 75% of those 65+ who are living in the community said they did not do anything they value or enjoy with their time NEWS TO SHARE Welcome Visits We would like to visit you to 1) Help our new team learn about community based services and understand the range and complexity of what you are doing- it will mean we commission more effectively 2) Help you get to know us and build relationships We sent everyone a letter- if you didn't get one and would like to offer time for a visit please let us know. Dementia Friendly Communities • Successful bid to be a National Pilot • working initially with12 communities across Kent • people will be aware of and understand more about dementia; • people with dementia and their carers will be encouraged to seek help and support; • and people with dementia will feel included in their community, be more independent and have more choice and control over their lives. Thank you Mark Lobban - Director Strategic Commissioning Emma Hanson - Head of Community Support Unit
