Medical forms and transportation check
are turned in at the main office of the
building the skier/boarder attends.

What about the weather? We plan on skiing every
Sunday (6) Mad River Mountain is open, starting with
Jan. 4th.
Exceptions:
Severe weather
MRM is closed
Conditions at MRM very poor

EMERGENCY PROCEEDURES
- staff member notified
- we call parent
- parent may be asked to pick up student
- student may be transported to Mary Rutan Hospital

- We bring injured student home on bus

5. Students must ride assigned
buses unless we (advisor)
okay a change.

their buses to and from MRM.
Failure to comply with their
requests could result in
suspension from the club

7. Prior to using any lift, you must

Emergency Medical Authorization Form
and the Mary Rutan Hospital Form.
These are the forms you were given tonight.

Send it in!
Deadline Nov. 19th
Return the two medical forms to school along with a check
made payable to Olentangy Local Schools or
Olentangy Ski Club
Check amount is $66.00 which covers the cost of
transportation. (Lakefront Buses)

 Using your smart phone or a camera, take a head shot
picture.
