Eligible -- 3.6 GPA by the end of first semester 2. Hand submit paper applications to Mrs. Hermane (2048) or Mrs. Miller (2057) no later than 3:15 PM (due date will be announced later) 3. Staff surveyed 4. Faculty council meets to approve applicants • Each section awarded points on a scale, all points are totaled and must average to at least 4.0 5. All applicants notified of acceptance status 6. NHS Induction on Thursday, April 10 @ 7:00 PM COMPONENTS OF THE PAPER APPLICATION 1. Cover Page • Full name, complete address, phone number and email address, current English teacher and room number, homeroom number, your signature in the bottom right corner 2. Leadership • List leadership positions in extra and co-curricular activities in a chart 3. Extra and Co-Curricular Activities • List all other activities and work experience PAPER APPLICATION CONTINUED 4. Community Service • List service, name of organization, hours, dates completed, brief detailed description • Total number of service hours • Include letters from sponsors with sponsors’ contact information 5. List of classroom teachers, advisors, and coaches • Include all teachers • If you did not attend BHS since freshman year, please note semester and year enrolled 6.
