equipped to deliver high quality, integrated care
closer to home
to devise measurable outcomes which
contribute valid, credible and empirical evidence
to inform knowledge and understanding about
planning and development of a motivated, fit for
purpose workforce
Objectives continued

project in order to evolve model of high
quality care which enables seamless
transfer between care settings and
integrated working between health, social
care, the third sector, service users and
carers at local PCT level and is transferable
across the Black Country region.
