Student’s First and Last Name, SID number,

School Name and Number at the top of the document.

Test Invalidation

If any of the following occurs:

1. Cheating or disruptive behavior

2. Illness during testing

3. Incorrect time

4. Incorrect preidentified information

5. Defective test/answer book

Write Invalid in red on the front cover of the test book.

Return with Not To Be Scored materials if test is invalidated

 Students absent on the regular administration date

 The date absent students make-up the test

 Students whose test books are invalidated

Keep a copy of the forms on file at your school

Test Administrator Responsibilities

Before Testing:

 Prepare room for testing by covering all visual aids

 Receive materials needed for testing

 Check student labels

 Complete ESE/504/LEP information (Sections 10 & 11) using page 7 from the Directions for Administering

During Testing:

 Distribute materials

 Read appropriate scripts VERBATIM (Directions Pages 9-13)

 Keep time

 Maintain test security

After Testing:

 Collect materials

 Check student grid sheets

 Complete the Administration Record

 Notify School Coordinator of any missing materials
