Standing in front of you is Alia Daniela P. Riego, student Nurse from BSN 1C, now ready to demonstrate the two significant procedures in a clinical set up for today. Explain the first procedure: First procedure that I will be doing is the SURGICAL HANDWASHING that is needed before performing a surgical procedure that aims to remove as many microorganisms from the fingernails, hands and forearms as possible before the sterile surgical procedure. 1. To formally start, gather the equipment needed including betadine soap, surgical scrub, paper towel, shoe cover, OR caps, surgical masks and googles. This is to save time and effort or even minimizes interruptions and allows for a smooth execution. 2. Remove all the jewelry to prevent harboring microorganisms. 3. Third, Be sure fingernails are short and clean, to help reduce the number of bacteria residing on nails. Next, wear a shoe cover, cap, mask, and protective eyewear. This provides protection against contamination. 5. Now, position on the sink with hands higher than the elbow and turn on the sink to help prevent splashing or contamination of the hands from dirty surfaces, ensuring that only clean water reaches the hands during washing. 6. And after that, I will be doing the handwashing up to elbow. Following the principle of moving from cleanest to dirtiest, thereby minimizing the spread of contaminants. 7. Next is to apply cleansing agent to brush or sponge since, Anti-microbial soap reduces microorganisms. 8. Wet hands and arms and keep hands above elbows. This is to ensure that movement of water and dirt will flow from hands to less clean areas, thus preventing contamination of the hands during the scrub. 9. Then, scrub the nondominant hand to ensure all surfaces will be systematically scrubbed scrubbed to remove transient microorganisms: a. Fingertips and fingernails for 20 circular stokes b. Thumb for about 5 downward stoke c. Inter-digitals for about 5 strokes each d. Dorsum with a 10 circular strokes f. Wrist to forearm (4 angles) for about 5 circular strokes g. Elbow for about 5 circular strokes 10. Rinse the brush. Put the liquid soap. Repeat step 9 to the dominant hand 11. Next, with hand higher than the elbows. Drops brush into the sink. 12. And then, rinse the first hand and arm from the fingertips to the elbow thoroughly with hands higher than the elbows. Following the same procedure to the next hand. This is to prevents contamination of the hands from dirtier areas. 13. Turn off the faucet. 14. Move to the sterile area to dry your hands. Explain the second procedure: Now that we are done in first procedure, we will be now proceeding to the Donning a sterile gown and gloves (Closed method). 1. Again, gather the equipment including sterile gown, gloves, cap, mask, shoe cover and a protective eye wear. This to save time and effort. 2. Ensure the sterility of the package 3. And then, observe appropriate infection control procedure. For the procedure of donning a sterile gown: 4. First, is to unwrap the sterile gown pack. The principle to this is outer surface of the gown remains sterile. 5. Second is to, open the package of sterile gloves. Remove the outer wrap from the sterile gloves, and leave the gloves in their inner sterile wrap on the sterile field. The rationale behind this is to keep the glove sterile and allow nurse who has scrubbed to handle sterile items. 6. Wear shoe covers, cap that covers the hair, face mask and protective eye wear to prevent those from contaminating sterile work areas, to protect mucous membrane of the eye, and covers the foot to reduce contamination from shoes. And then, perform the hand hygiene which is the surgical scrub to prevent spread of microorganisms. 8. Put on sterile gown. Grasp the sterile gown at the crease near the neck; hold it away from you, and permit it unfold freely without touching anything, including the uniform. This is to maintain sterility of the gown and positions it for donning. 9. Put the hands inside the shoulders of the gown, and work the arms partway into the sleeves without touching the outside of the gown. Work the hands down the sleeves only to the proximal edge of the cuffs. Fingers remain in the cuffs to protect the sterility of the gown and prepare for closed gloving. 10. After that, have a coworker grasp the neck tie without touching the outside of the gown, and pull the gown upward to cover the neckline of your uniform in front and back. (Gowning continues at step 21). Maintains sterility of the gown. Gowns are considered sterile in the front from the shoulder to the table level, and sleeves are considered sterile from 2 inches above the elbow to the wrist. Next thing to do is the procedure of donning sterile gloves in a closed method. 11. Open the sterile glove wrapper while the hands are still covered by the sleeves. This is to maintain sterility of the gloves. With the dominant hand, pick up the opposite glove with the thumb and index fingers, handling it through the sleeve. Same rationale, to maintain sterility of the gloves. 13. Lay the glove on the opposite gown cuff, thumb side down, with the glove opening pointed toward the fingers. Position the dominant hand palm upward inside the sleeve. SAME 14. Next is to, Use the non-dominant hand to grasp the cuff of the glove through the gown cuff, and firmly anchor it. Note that sterile touches sterile. 15. With dominant hand working through its sleeve, grasp the upper side of the gloves cuff, and stretch it over the cuff of the gown. 16. Pull the sleeve up to draw the cuff over the wrist as you extend the fingers of the non- dominant hand into the gloves finger. This is to ensure that the glove is worn correctly and securely. 17. Put the glove on the non-dominant hand. Place the fingers of gloved hand under the cuff of the remaining glove. This is to ensure that the glove is worn correctly and securely. 18. Place the glove under the cuff of the second sleeve helps create a secure barrier, minimizing the risk of exposure to contaminants and maintaining a sterile environment during procedures. 19. Extend the fingers into the glove as you pull the glove up over the cuff, to ensure that it has full dexterity. Have a coworker take the two ties at each side of the gown, and tie them at the back of the gown, making sure that your uniform is completely covered, ensures that your uniform is fully covered and protected. 21. When worn, sterile gowns should be considered sterile in front from the waist to the shoulder. The sleeves should be considered sterile from 2 inches above the elbow to the cuff since the arms of a scrubbed person must move across a sterile field, ensuring aseptic conditions. 22. Remove protective devices: remove gloves first, then the mask, gown, the eye wear or goggles, cap and shoe cover, minimizes the risk of self-contamination and exposure to pathogens.
