Project Title: Project MATAPANG ( Monitoring and Tracking of students Academic Performance and Need for Guidance) Problem Statement: BALNASAN NHS has not develop a comprehensive academic monitoring and guidance system for transitioning from grade 10 to grade 11, utilizing data to track individual students performance, identify areas of improvement, and provide timely guidance and resources to support their successful transition and academic achievement. Project Objective: To create an effective academic monitoring and guidance program that will enable the seamless transition of grade 10 students into grade 11 by tracking their academic performance, specific needs for guidance, and providing targeted interventions and resources to enhance their academic success. Root Cause: Lack of clear expectations, Learning Gap, Personal Challenges, Limited Guidance, Inadequate Resource, Study skills and Time management, Motivation and Engagement, Child Labor.
