SBAR assignment Situation: The client is exhibiting nonverbal signs of discomfort and was more anxious than usual. Nonverbal indications observed while bathing was increased irritability, keeping arms tightly clung to the chest, and inability to soothe like in previous interactions. During hygiene care on 11/03/2022, there was a semi-circular rash on the lateral side of the RLE about 0.5" in diameter. Background: The rash was visible on 11/03. I notified the nurse, and the occurrence was documented. On 11/10, during hygiene care, the irritation was still present, was 1.5" in diameter, and a complete circle formed. On 11/17, the rash was 3" in diameter. Three new formations were observed on her LLE lateral right below the greater trochanter. Assessment: My assessment of this situation is that the patient may have ringworm, a fungal infection that has gone systemic. Recommendation: I have informed the nurse manager and the doctor directly of this occurrence, and a topical antifungal treatment has been prescribed. This topical medication must be applied bid.
