INCIDENT REPORT

Dear Sir,
Greetings!
This is to inform you about the Nutritious Food Commodity particularly the Chicken Arrozcaldo was
received last February 27, 2023, Monday at the drop off point Mawab Central Elementary School
Sped Center, Mawab, Davao De Oro.
Tabontabon Elementary School received 56 pcs of chicken arrozcaldo together with other food
commodities. On the day of the scheduled feeding of the arrozcaldo (Thursday), I opened 28 pcs
of arrozcaldo for all the beneficiaries of that day. I smelled and tasted each packs to ensure its
quality, it was good and I gave it to the beneficiaries. Later that day, I read messages from our
group chat saying that their arrozcaldo smell bad and spoiled, so some of them didn’t give the
foods to their beneficiaries. Also, through the group chat, our District Nurse announced that all
remaining arrozcaldo will be returned back to the Division.
With this, I monitored my beneficiaries, asking them if they feel uneasiness in their stomachs or
are they having stomach aches. Gratefully, the beneficiaries felt okay even the day after they have
eaten the arrozcaldo. Furthermore, 28 pcs of remaining arrozcaldos were sent back to the drop off
point, Mawab Central Elementary School Sped Center, Mawab, Davao De Oro.
