I am giving my son/daughter ______________________________________ the permission to attend
the Practical Exam (Walking Tour) under TOU 058 – Tour Guiding subject on October 14, 2022
for Batch 1, 2 and 3 and October 21, 2022 for Batch 4,5,6.
Furthermore, I am aware that there is some risk connected with the above-mentioned activity,
but I am convinced that my permission for my son/daughter to undergo such activity will surely
go a long way to his/her educational formation. Therefore, in view of the above, I shall not hold
PHINMA – University of Iloilo, its Officers, Board of Directors and the faculty members liable for
any untoward incident that may arise on account of my son/ daughter’s failure, fault, or
negligence, and of causes purely accidental, fortuitous, or beyond control. Hence, I have been
made aware that the establishment is not obliged to provide remuneration for the services.
