OJT REQUIREMENTS
1. Parent’s Consent - Print back to back, 3 notarized copies
2. Deed of Undertaking – Print back to back, 3 notarized copies
3. Medical.Certificate
4. Psychological Certificate – to be followed, since the test hasn’t been conducted yet
5. Photocopy of Parent’s ID with 3 signatures in blue ink
6. Application letter
7. Resume with 2x2 picture
8. Photocopy of Philhealth ID or MDR
9. Photocopy of Vaccination Card
10. Waiver
Put your requirements in brown envelope for shape keeping of documents. DO NOT WRITE
ANYTHING ON THE ENVELOPE.
WHAT TO DO IF YOU CHOOSE TO APPLY FOR OJT IN A NON-PARTNER COMPANY?
1) The non-partner company must send a letter to the University stating that they are willing to
accept OJT students and become a partner of the University.
The company must also submit the following:

3) Company Profile
Submit the documents to Ma’am Showna in hardcopy.
ADDITIONAL NOTES AND INSTRUCTIONS
a. Reach out to Ma’am Showna for available Letter of Intent (LOI).
b. Upon acceptance to a certain company, submit an Acceptance Letter (signed by the
company’s head) to Ma’am Showna.
c. For the Memorandum of Agreement (MOA), the student representative per company
must email (or personally reach her out) Ma’am Showna to have a copy of the
document.
