Pre checking
All PIC’S must provide
workers/vendors name list
coming new factory and state
the location of job (ex:1st,2nd
floor).
All the work must issue the
PTW by (Initiator/PIC’s).
Copy of PCR test attached
together with PTW form.
PTW valid for 1 DAY
WORKING ONLY.
Entering new Factory
1. All PIC’S and workers/vendors
must wait at the guard house.
2. Show the PCR test to security.
3. PTW completed by initiator.
4. Safety will check and verify PTW
-Safety Briefing
-PPE requirement (Safety helmet,
safety vest, safety shoes)-Must wear
depend on job condition.
-Safety Barricade- Must bring your own
safety cone or any type of barricades.
-Hang Up the PTW at work area
