It is required that this Addendum Number 1 be signed in the Acknowledgment of Receipt below, and
attached to the Proposal and become part of the Proposal.

This Addendum includes the following:
1. DELAY OF BID OPENING:
The bid opening is being rescheduled from September 2, 2015 to Friday, September 4, 2015 at 2:00
p.m.
2. CHANGES TO THE DRAWINGS AND SPECIFICATIONS:
None
3. RFIs FROM PROSPECTIVE BIDDERS:
3.1 The “Bid or Proposal Bond” (BF 15 of 46 and 16 of 46 in the bid documents) has a place for our
Bonding Company to sign, however, there is not a place for Bidder to sign as is typical on any other
bond form. Is our signature required?
Only surety is required surety and/or insurance agent are required to sign Bid or Proposal
Bond BRAA Form BB001
3.2 The “Statement of Good Faith Efforts” BRAA Form DBE004 is not contained in the bidding
documents or found on the BRAA website. Could you forward that form to Bidders.
DBE SUBCONTRACTOR AND SUPPLIER SOLICITATION SHEET must be completed for
each DBE subcontractor and supplier solicited. It is separate from the Statement of Good Faith
Efforts. The DBE004 is attached to the addendum.
3.3 On drawing C006 - NOTE #2 allows 10 days of night work to complete edge drop-off repairs.
Considering the amount of clearing, amount of imported fill and topsoil needed, the amount of sod
and placing and removing of low profile barricades each night, this time appears to be inadequate.?
An additional 10 days of work will be allowed for a total of 20 calendar nights of work to
complete edge drop off repairs. Additionally, The BRAA will consider requests for time above
the 20 nights if no additional costs above the line items/unit costs are requested, and the time
does not exceed that allowed for the adjacent EMAS construction project.

 Staging area was discussed- shown on plan sheet(s) outside of the fence.
 Airfield traffic control was reviewed for the shoulder and the drop-offs. Shoulder will have a
permanently closed area for the duration of the work in that area. Drop-off area closure was described
according to the plans.
 Latest FAA Advisory Circular for Safety will be adhered to during construction.

Construction Summary
 Shoulder Paving
 Daytime Work.
 Traffic Control will consist of permanent closure for the duration.
 Concrete Pavement. Materials Specifications are FDOT standard.
 Drop-off Repairs
 Nighttime Work.
 Runway will be closed during the work and re-opened in the morning.
 Penalties for not re-opening the runway (described in the bid documents).
 Aircraft may be moving around airport at night under their own power or by tug.
 Areas will be identified in field by OAR, usually a week before the work to assist planning and
airport coordination.
 Protect burrowing owls according to the plan details.
 Payment by truckload of material. Tickets and visual verification will need to occur with OAR.
 Lighted X
 Contractor provides the equipment for this project and leaves at airport upon completion.
 Contractor will not be responsible for placing and maintaining the equipment when the EMAS
project needs a closure, but this contract does not. If there is a closure needed for both
projects, this Contractor for this bid will need to provide the equipment and maintenance for that
night.
 Badging
 Everyone will need to be badged. Cost is $25. Need to provide a 5 year background check.
 Concrete trucks can be escorted and do not need to be badged.
Adjacent Runway EMAS Project
 Bids are open at this point. Apparent low bidder has not yet been identified.
 Mobilization will begin late September- early October.
 Ideally both projects would be completed by Thanksgiving.

STATEMENT OF GOOD FAITH EFFORTS
Project: Holding Pad Shoulder and Pavement Edge Drop-Off Repair
BIDDER:
This form is to be completed if BIDDER fails to achieve the DBE goals established for this Work.
The BIDDER is allowed to use an alternate method that demonstrates the good faith efforts made to
meet the goals established as long as all of the requested information is included. Failure to include
all requested information shall result in the Bid being determined as nonresponsive to the DBE
requirements.
BRAA will look not only at the different kinds of efforts the BIDDER has made, but also the quality,
quantity, intensity and timeliness of those efforts. It is the responsibility of the BIDDER to exercise
good faith efforts. Any act or omission by the BRAA shall not relieve the BIDDER of this
responsibility.
Criteria listed below are excerpted from Appendix A of 49 CFR93, as amended. A response is
required to address each cited paragraph. Additional pages may be added as necessary.

perform the type of work to be subcontracted and advising the DBE’s of the specific work the

All letters from BIDDER to prospective DBE subcontractors must be post marked or fax
recorded a minimum of fifteen (15) calendar days prior to bid opening.

to their calls and project information they received from the BIDDERs.)

including, where appropriate, breaking contracts or combining elements of work into
feasible units. The ability of the BIDDER to perform the work with its own work force
will not in itself excuse a BIDDER from making positive efforts to meet the established
