PLEASE FOLLOW THE INSTRUCTIONS BELOW TO
DICTATE!
1. If you are in the hospital dial x—4444; if you are outside of the hospital you can dial (718)
470-4444.
2. Enter #6 then your physician number followed by the pound (#) key.
3. Enter the Medical Record Number followed by the pound (#) key.
4. Enter the location where the procedure was done followed by the pound (#) key:
1—Forest Hills
2—Plainview
3—Glen Cove
5—Manhasset
6—LIJ
7—Franklin
9—Syosset
5. Enter your work type followed by the pound (#) key:
1. Operative Reports
2. Discharge Summaries
6. Enter the 6 digit date for the date of the procedure (MM/DD/YY)
7. Begin dictation.
8. A job confirmation number will be assigned to each dictation. Press # and please note this
job confirmation number in the chart as backup to your dictation.
9. If dictating more than one patient, you must press 6 on the keypad to indicate next work file
or another job number will not be assigned. Then repeat the above process.
10. An incomplete dictation can be “Marked Open” by pressing 0 on the keypad.
