EL CAMINO COLLEGE LCM TCU CLINICAL ROTATION TCU ROUTINES HANDOUT The unit uses a functional nursing delivery system with the following assigned roles: o Treatment nurse - does all the dressing changes o Medication nurse – administers all the medications except via IV infusion route o Intravenous nurse that gives all the IV medications and fluids. o Charge nurse, who sits at the desk and is in charge of assessments and contacting the physicians for orders or problems, o CNAs – ADLs for all patients. Other services present: Respiratory Therapist (RT) Physical Therapist (PT), Occupational therapist (OT, Speech), Dietician, and Case worker. Vital signs to be completed by 0800 and documented in a unit flow sheet for physicians to reference. Routine medication administration times: Daily at 0900; bid 0900 & 1700; tid 0900, 1500 & 1800. Diet, BM, and pain are documented on the medication sheet q shift. Caterers bring in food at 0800, 1230, and 1700. ac finger sticks for lunch and dinner are to be done at 1130 and 1600. Special instructions regarding patient care are written on the white board behind the patients’ head of bed. Examples: Activity levels, any patient limitations, NPO, special instruction on feeding a patient, what arm not to use for taking BP etc. Patients’ assessment sheets (both daily and weekly) and VS flow sheets are kept in individual patient files.
