The Peralta District Benefits Office announces the following update to the Interplan Network
Change for current CoreSource Members. If you are currently a Kaiser Member through
Peralta or if you are not insured with Peralta, you may disregard this Update.
As announced in the August Benefits Office Update, and again in the mailing to current
CoreSource participants, revised insurance membership cards integrating the Blue Cross
Network references were mailed out last week.
To ease transitional anxiety for those who may not have received a card yet, take a moment to
read important changes which will be noted on the new cards as a result of our return to the Blue
Cross medical network. The Benefits Office continues to track the commonly asked questions
and hopes you find the following information useful:

I have not received my card yet. What
can I do in the meantime?
Where does my in-network provider send
claims?
Answer
If your service provider is in the Blue Cross network and wants to
verify your eligibility over the phone, continue to use the benefit
eligibility number 1 866 280 4120 noted on your current card.
Prudent Buyer Plan Network Providers should mail claims to:
Post Office Box 60007 Los Angeles, CA 90060 0007
Out-of-Califorinia BlueCross/Blue Shield participating providers
should mail claims to their local claims office. Blue Cross medical
providers agree to follow billing protocols when they become part
of the Blue Cross Blue Shield Network.
How can I find out if my provider is in the
network?
1. Ask your current provider-they usually know their network
affiliations
2. Look online
 inside California: bluecrossca.com
 outside California: bluecares.com
3. Call Coresource-that’s right. CoreSource customer service
can assist in locating providers: 1 866 280 4120
What else is different on the card that I
should advise my service providers?
Due to increased protection of consumer privacy and health
records, each family unit will have unique identifier other than
the SSN. Other numbers appearing on member cards:
CoreSource Group #:4138-no change
Blue Cross identifier for California residents: 175182M001
Blue Cross Identifier outside of California: 175182M002
