This evaluation form is due no later than 11 a.m. on Wednesday, May 2, 2007.

Take a few minutes to carefully evaluate each member of your group (including yourself). Your focus should be based on their
overall performance on the semester project and their contribution to the group. This is not a personality contest – your
evaluation should not be based on whether you like or dislike an individual. Evaluate each member’s project performance.
YOU MUST INCLUDE YOURSELF IN THE EVALUATION! Please read and follow all of the instructions carefully.
You must complete all sections (both pages) of this evaluation! You may submit this electronically or in person.
A. Please list the names of all of your group members (include yourself) IN RANK ORDER (that means the person you feel
contributed the most should be listed in #1, second highest contributor as #2, etc.):

possible score and a 10 is the highest rating. If you believe that all members did an equal amount of work, all members

C. Now, multiply the number of people in your group by 100 points. Then distribute these points and place them beside the
persons’ named in Section A above (you must include yourself in this distribution of points). You must use all the points
and you cannot give anyone less than 80. If you were, for example, to give someone an 85, you must distribute the
remaining 15 points to other group members (these points can be given to 1 group member or distributed among several
group members). Remember, when you add up you points, they must equal the number of people in your group times 100
(for example, if you there are 5 people in your group you have 500 points to distribute).
D. So that I can better understand your quantitative evaluation, please provide your written comments about your group
project experience. I want you to comment on EACH person’s effort (by name) in this project (including your own).
Your written comments will be kept confidential. YOU MUST COMPLETE THIS SECTION, Otherwise your
evaluation will not be considered complete.
