Patron Confidentiality Waiver
I understand that according to 43.30(1) of the Wisconsin Statutes, records indicating the
identity of individuals who borrow library materials may not be disclosed to anyone
outside the library administration, except by court order.
I further understand that by signing this waiver I am giving up any expectation that
my records relating to any audio or video equipment borrowed from the Journalism
Reading Room will be kept confidential.
Therefore, I give permission for employees of the Journalism Reading Room to release
any records relating to audio or video equipment, to any employee of the UW Journalism
Department or their agents.
I further understand that my statutory right to privacy will not resume until all audio and
video equipment I borrow during this semester is returned.
