Creating Catholic Conditions for Well-Being, Learning and Leading
Outcomes:
By June 2016, there will be an increase in the students feeling less bullied as demonstrated
in the CCCC School Climate Survey results.
Outcome Progress Comments
5/31/2015: St. Bernard of Clairvaux continues to work towards helping all students feel safe and included so that
they will feel less bullied. Data from the upcoming School Climate Survey will inform practice and determine next
steps, but we have focused on preventing bullying by fostering and nurturing a school community that is safe,
caring, healthy and inclusive. We have done this by engaging students in monthly bullying lessons and providing
students with opportunities to be Peer Mediators, Eco-Reps, Youth Faith Ambassadors, Kindergarten helpers,
Library helpers, Office Helpers and Intramural leaders. We have also provided students with opportunities to listen
to presentations from guest speakers and our Peel Police Education Officer focusing on the theme of bullying. We
also continued to support initiatives such as our PALS program (Playground Activity Leaders in Schools), Virtues
Assemblies, and the WITS program (Walk Away, Ignore, Talk it Out, Seek Help) to support all students in our
ongoing efforts to sustain a safe, caring, healthy and inclusive school environment for all.

If we are more intentional in creating respectful conditions for well-being, learning and
leading, in a positive climate, then by June 2016, parish and school community will feel
included, informed and engaged as demonstrated by observation, conversation and voice.
Outcome Progress Comments
5/31/2015: As a school community, St.Bernard of Clairvaux continues to foster positive school-community
partnerships. We continue to work with the Public Library, Peel Health, Peel Police, the Early Years, YMCA and the
St. Josephine Bakhita parish community. We continue to engage our school community in events such as our Open
House/BBQ, Christmas Concerts, Junior Showcase, Diversity Night, Welcome to Kindergarten and Spring Fling. We
have continued to strengthen the home-school-parish relationship through the preparation of the sacramental
retreats and celebrations. Our Adult and Youth Faith Ambassadors work together to support adult and student
faith formation in addition to social justice initiatives. We continue to celebrate mass as a school community and
we pray the Rosary during the months of November and May. St.Bernard of Clairvaux continues to be a faith-filled
community centered on peace, love, service and excellence.

Building Collaborative Practices Through Inquiry
Outcomes:
By June 2016, there will be an increase in the proportion of educators initiating
opportunities for collaborative practices as evidenced in learning activities.

5/29/2015: At St. Bernard of Clairvaux, our staff has been provided with many opportunities for collaborative
practices. We have fostered collaborative practices with staff by honouring staff voice in divisional meetings, staff
meetings, job-embedded professional learning opportunities, PA days, informal professional dialogue as well as
lunch and learn meetings. Staff have continued to use common planning time to collaborate on common teaching
and learning strategies that support all learners. In addition, teachers have also used recess, lunch and after-school
opportunities engage in collaborative practices. This has also manifested itself with our students, where a focus on
learning skills such as collaboration has resulted in more positive collaborative efforts between students.

5/31/2015: St. Bernard of Clairvaux staff continues to provide students with many opportunities to collaborate in
and out of the classroom. Students have collaborated with their teachers in co-constructing learning goals and
success criteria in content areas as well as in their learning skills. They have also worked with their teachers and
peers to provide timely, descriptive feedback to each other to support their learning. Students have also
demonstrated opportunities to collaborate with peers in 3 part Math lessons as well in cross-curricular inquiry
projects. The students of St. Bernard have also been provided with opportunities to collaborate and develop
leadership in school-wide initiatives such as PALS, Peer Mediators, Youth Faith Ambassadors, Intramurals (sports,
robotics), and Eco-Friendly Schools. We will continue to focus on learning skills such as collaboration so that our
students continue to develop the ability to work with others.

Enhancing Transitional Practices
Outcomes:
By June 2016, there will be an increase in successful practices that support transitional
practices, as demonstrated in student-staff-parent and community observations and voice.
Outcome Progress Comments
5/29/2015: St.Bernard of Claivaux school staff continues to engage in many successful practices that support
transitional practices. Staff, support services staff and parents work in collaboration to support all learners with a
variety of transitions. For example, we have collaborated with our feeder school, St. Joan of Arc, through
conferences, to discuss grade 8 students at-risk and plan for individual transitions into high school. We continue to
use practices such as social narratives, pictures, first-then strategies, choice boards, peer buddying, classroom visits
in August and throughout the year, and community engagement evenings (Welcome to Kindergarten) to support
our learners with transitions while at school. We continue to support our students who need to transition to
different school communities for programs such as French Immersion, Extended French and Gifted by providing
information to students and parents regarding information evenings and coordinating visits to the schools. By
working collaboratively with all partners in education, we continue to successfully support our students with the
many transitions that they may experience.

Outcomes:
By 2016, there will be an increase in successful practices (Guided instruction/Reading, 3part Math lesson, formative assessment) that support knowing the learner as demonstrated
in professional learning feedback, student/staff/parent/guardian/community observation,
conversation and voice.
Outcome Progress Comments
5/29/2015: The staff of St.Bernard of Clairvaux has engaged in many successful practices (guided
instruction/reading, 3-part math lesson, assessment for/as learning) that support knowing the learner. School staff
have used practices such as student intervention meetings, creating class profiles, differentiated instruction as well
as timely, descriptive feedback related to learning goals and success criteria to gain a deeper understanding of
each learner. Educator learning continues to deepen as demonstrated through professional dialogue with
colleagues, and exit tickets shared with staff and administration regarding pedagogical practices to support the
process of knowing the learner. Educators at St. Bernard of Clairvaux have demonstrated their growth as learners
and reflective practitioners as demonstrated in professional learning feedback.

Outcomes:
By June 2016, staff will utilize effective instructional practices (Guided instruction/Reading,
high yield strategies, 3 part Math lessons) in a more personalized and responsive way to
support students' needs.

5/29/2015: The staff of St.Bernard of Clairvaux continues to utilize effective instructional practices (guided
instruction/reading, high yield strategies, 3 part math lesson) in a personalized and responsive way to support
students' needs. By generating a class and student profile, teachers are able to differentiate instruction and
assessment to meet the needs of individual learners. Student intervention meetings with staff have supported a
more personalized approach to the use of strategies suited to the learning styles of students. In addition, a focus
on personal, descriptive feedback aligned to learning goals and success criteria has provided opportunities for staff
and students to clearly understand what is done well, what still needs to improve, and the next steps for growth.
The staff's commitment to getting to know the learners on a deeper level has supported a personalized approach
to meeting the needs of the learners in their care.
