This document contains two Geriatric Medicine placement descriptions: North Central Thames Foundation School (NCTFS) Individual Placement Description - East and North Herts NHS Trust Placement The department The type of work to expect and learning opportunities Location Supervisor(s) Main duties of the placement Typical working pattern in this placement F2 Geriatric Medicine Elderly Medicine Department All F2 Doctors in hospital posts will generally be ward based during the ‘normal’ working day and expected to deliver the daily medical care of all the patients on their ward. Whilst on call, the F2 will be involved with the clerking of medical patients being admitted and the on-going care of the patients in the medical wards irrespective of the specialties The overall educational objectives of the F2 year are to provide the trainee with the knowledge, skills and attitudes to be able to Take a detailed history and conduct thorough examination of patients Identify and synthesise problems Prescribe safely Keep an accurate and relevant medical record Manage time and clinical priorities effectively Communicate effectively with patients, relatives and colleagues Use evidence, guidelines and audit to benefit patient care Act in a professional manner at all times Cope with ethical and legal issues which occur during the management of patients with general medical problems Educate patients effectively Become life-long learners and teachers Involve in audits and departmental teaching Attend frailty clinics Experience in community long term conditions. Opportunity to attend nursing homes and observe community working – consultant led. Lister Hospital Dr S Wijyasiri, Dr A Malik The F2 doctor is responsible with other staff for the ward care of patients and the maintenance of the patient’s medical record. They also work with the consultant in out-patients clinics – there is opportunity to attend daily Rapid Access Clinics for older patients. They are expected to work closely with other members of the MDT. They are expected to organize and attend the structured teaching programmes provided by the department. The doctor will also be responsible for such other specific clinical duties as allocated by consultants. They also work with the consultant in out-patients clinics – there is opportunity to attend daily Rapid Access Clinics for older patients. The F2 doctors will be doing consultant / GP led visits to 3 nursing homes and 2 community rehabilitation units. They will also be opportunity to work with community based multidisciplinary rapid response team with the consultant. They are expected to work closely with other members of the MDT. They are expected to organize and attend the structured teaching programmes provided by the department. The doctor will also be responsible for such other specific clinical duties as allocated by consultants. Daily am handover & pm board meetings. Consultant led ward rounds 3 times a week Mon – Fri: Ward work and ward round Consultant / GP led nursing home ward rounds (3/ week) Consultant led round in community rehabilitation units (2/ week) Consultant led domiciliary visit to community based rapid response multidisciplinary team.
