ב"ה MICHIGAN JEWISH INSTITUTE Policy and Procedure Manual Functional Area: Title: RECORDS MANAGEMENT Policy No. RM 1105 Completing Registration Forms Effective Date: May 2001 Page No. 1 of 2 GENERAL PURPOSE: This policy is intended to provide a process and procedure that will insure the correct completion of registration forms by students and the correct entry of same into students’ electronic and hard copy record of courses and grades (a.k.a. grade record or transcript.) POLICY: It is the policy of MJI that there be 100% consistency in course names, course numbers, identification of course names and numbers on registration forms, and entry of the same into student records (electronic and hard copy). All MJI employees responsible for student registrations and/or data input into student records will be oriented on this policy and procedure and will make it part of their records management activities. PROCESS: To insure that all course registration forms are accurately completed by students and entered accurately into MJI electronic and paper files, the following procedures are to be followed: 1. No later than one week prior to the opening of a specific semester’s registration, office staff should prepare course registration forms that contain all course offerings by name, number and credits available for that semester. Preparation of these forms is to be crosschecked against the most current MJI college catalog for verification of correct course names and numbers. 2. In addition to the standard courses to be offered, the registration form should include an area for the completion of directed studies requests. Registration forms containing requests for directed studies must also have attached, approved learning contracts for the directed studies requested. Students should be alerted on the form that MJI staff would not accept registration forms that include inaccurate or incomplete directed studies requests. MJI staff are not authorized to accept any registration form from students that is incomplete. Therefore, staff should audit forms as students turn them in. Should a student choose to mail a registration form to MJI or otherwise submit it without staff contact and review, upon determination that such a registration form is incomplete, it is to be immediately returned to the student via mail or other means for correction. 4. Data entry of information taken from registration forms is to be conducted only by MJI staff appropriately trained for this activity. Due professional diligence is expected so that registration records, grade records, transcripts and other documents will all contain consistent course information. 5. This policy is effective immediately upon its authorization and publication. A model registration form has been attached to this policy for use as needed.
