(Self-evaluation, to be completed by student)
All social work students must complete at least 400 hours of field experience. To best evaluate and measure student progress over time, this evaluation should be completed twice during the placement
(once at about the halfway point and once upon completion of total hours). It is important to your growth and development that you self-evaluate. That way, you can identify areas of strength and weakness and strive to improve your practice. This evaluation is also an important part of the UWSP Social Work
Program’s assessment process; we use data generated to identify areas in need of improvement in our curriculum. For your own benefit, and for that of our program, please complete this evaluation carefully and honestly.
