PERSONAL DATA-PRIVACY ACT OF 1974 Direction from SMSgt Chang: They need to bring their form to any of us with a copy of their orders and FRF/FEF. Direction from TSgt Clemente-Musser: Usually, if the member hand-carries their record to Monterey, they fill out the in-processing checklist and then the courier brings their records to us. If the members record is mailed from their last base to us--there will be no checklist so we will need them to fill one out once they get to Monterey. Then if they can fax it to us that would be perfect. DSN 424-1031. V/r, TSgt C-M TSgt Melissa M.
